目的 :观察宫颈细胞DNA与液基细胞检查在宫颈癌早期筛查中的应用效果。方法 :取宫颈癌早期筛查患者12630例,入院后患者均行宫颈细胞DNA倍体定量检测和液基细胞学联合测定完成早期筛查。对1146例宫颈癌细胞DNA倍体定量测定或液基细胞学筛查阳性患者进行阴道镜检查并进行组织病理学检测。结果 :1728例DNA定量分析阳性,占13.68%;1133例液基细胞学检查阳性,占8.97%。DNA定量分析中10902例未见,1063例DNA细胞少量异常;液基细胞学检查中11497例正常或炎症;575例为不典型鳞状上皮细胞,365例低度鳞状上皮内病变;1146例液基细胞学检查DNA定量分析异常行阴道镜检查活检,5阳性率为50.61%,诊断敏感性为89.14%(517/580),特异性为75.80%(429/566);1070例液基细胞学检查阳性,以宫颈活检为标准,液基细胞学检查敏感性为73.28%(425/780),特异性为76.50%(433/566)。结论 :液基细胞检查与宫颈细胞DNA定量分析均为早期宫颈癌常用的筛查方法,且两者联合筛查能提高诊断敏感性、特异性,值得推广应用。
Objective: To observe the effect of cervical cell DNA and liquid-based cells in early screening of cervical cancer. Methods: Totally 12630 patients with early cervical cancer screening were enrolled. Conclusion: Liquid-based cytology and quantitative DNA analysis of cervical cells are commonly used screening methods for early cervical cancer. Combined screening of the two can improve the sensitivity and specificity of diagnosis and is worth popularizing and applying.
