目的观察经颅多普勒超声(TCD)对颈部和颅内血管狭窄诊断的可靠性。方法收集患者20例,分别用2mHz和4mHz,多普勒探头检查颅脑和颈部大动脉的血流速度。然后做数字减影血管造影(DSA)。结果在闭塞性脑血管病组共检查血管300条,多普勒超声发现狭窄血管85条,215条正常,其中73条与DSA所见一致,DSA发现另外12条狭窄血管。统计结果显示TCD诊断血管狭窄的敏感性是很高的。结论 TCD对诊断颅内和颈部严重狭窄血管据有较高特异性和敏感性。对大脑中动脉、颈内动脉近远端以及锁骨下动脉狭窄的可靠性较高,不易与其他疾病相混淆。可作为DSA检查的筛选手段。
Objective To observe the reliability of transcranial Doppler ultrasound (TCD) in the diagnosis of cervical and intracranial vascular stenosis. Methods Twenty patients were collected, and the blood flow velocity of the brain and neck aorta were examined with 2mHz and 4mHz Doppler probes respectively. Then do digital subtraction angiography (DSA). Results A total of 300 blood vessels were examined in the patients with occlusive cerebrovascular disease. 85 stenotic vessels were found by Doppler ultrasound and 215 normal. Among them, 73 were consistent with DSA findings and another 12 were stenosed by DSA. The statistical results show that the TCD diagnosis of vascular stenosis is very high sensitivity. Conclusion TCD has high specificity and sensitivity for the diagnosis of severe stenosis of the brain and neck. The middle cerebral artery, internal carotid artery near the distal and subclavian artery stenosis higher reliability, not easy to be confused with other diseases. DSA screening can be used as a screening tool.
