目的观察急性脑卒中患者心律失常发生情况及进行心率变异性分析。方法对82例急性脑卒中患者及40例健康对照者进行24h全信息动态心电图检查,观察心律失常发生情况及进行心率变异性分析。结果急性脑卒中患者心律失常新发率较高、且可出现严重的室性心律失常,并与对照组相比SDNN、RMSSD、SDNNindex、SDANN、PNN50(%)差异均有非常显著性意义(P<0.01),急性脑卒中患者心率变异性的昼夜变化则SDNN、RMSSD差异无显著性意义(P>0.05),而SDNNindex、SDANN差异有非常显著性意义(P<0.05)。结论急性脑卒中患者交感神经兴奋性增加、迷走神经兴奋性降低,且昼夜变化规律性消失,心率变异性降低,警惕有猝死的可能。
Objective To observe the incidence of arrhythmia and analyze the heart rate variability in patients with acute stroke. Methods A total of 82 patients with acute stroke and 40 healthy controls were subjected to 24-hour Holter Holter monitoring. The incidence of arrhythmia and heart rate variability were analyzed. Results The incidence of arrhythmia in patients with acute stroke was high, and severe ventricular arrhythmia occurred. Compared with the control group, the differences of SDNN, RMSSD, SDNNindex, SDANN and PNN50 (P <0.05) were significant (P <0.01). There was no significant difference in the diurnal variation of heart rate variability in patients with acute stroke between SDNN and RMSSD (P> 0.05). However, there was significant difference between SDNNindex and SDANN (P <0.05). Conclusions The patients with acute stroke have increased sympathetic activity and decreased excitability of the vagus nerve. The regularity of diurnal variation disappears and the variability of heart rate decreases, so it is possible to guard against the sudden death.
