目的:探讨脑胶质瘤组织中八聚体结合转录因子4(OCT4)表达情况及其与患者预后的关系。方法:收集南阳市第二人民医院2015年3月至2016年6月收治的脑胶质瘤患者168例和颅脑损伤术后手术留取的正常脑组织标本72例。采用反转录-聚合酶链反应(RT-PCR)检测标本组织中OCT4A、OCT4B mRNA表达水平,分析其与脑胶质瘤患者临床病理特征及预后的关系。结果:168例脑胶质瘤组织中OCT4A、OCT4B mRNA相对表达量分别为(2.28±0.85)、(2.84±1.29),均明显高于正常脑组织的(1.05±0.41)、(1.18±0.46),差异均有统计学意义(n t=11.649、14.798,均n P<0.01);脑胶质瘤组织中OCT4A与OCT4B mRNA表达水平呈正相关(n r=0.682,n P=0.001)。不同病理分期、病理类型、分化程度、浸润深度的脑胶质瘤患者OCT4A、OCT4B mRNA表达水平差异均有统计学意义(n t=14.695、11.309,16.038、13.721,17.216、15.083,14.871、15.417,均n P<0.01)。以OCT4A、OCT4B mRNA表达的中位值为界值,分为低表达、高表达,OCT4A、OCT4B低表达患者总生存期分别为(24.17±3.41)个月、(25.30±4.16)个月,均明显长于高表达患者的(15.80±2.93)个月、(15.94±3.07)个月,差异均有统计学意义(n t=15.639、16.143,均n P=0.000)。Cox回归模型分析显示,OCT4A、OCT4B表达水平是脑胶质瘤患者预后的独立影响因素,风险比(95%n CI)分别为1.731(0.804~3.181)、1.605(0.795~2.942)。n 结论:脑胶质瘤组织中OCT4A、OCT4B表达水平异常升高,且与患者病情严重程度密切相关;OCT4A、OCT4B高表达的患者总生存期较短,对脑胶质瘤的治疗效果和预后评估具有一定的预测价值。“,”Objective:To investigate the expression of octamer-binding transcription factor 4(OCT4) in glioma tissues and its relationship with prognosis of patients.Methods:From March 2015 to June 2016, 168 patients with glioma and 72 normal brain tissues in the Second People's Hospital of Nanyang were collected.The expression levels of OCT4A and OCT4B were detected by reverse transcription-polymerase chain reaction(RT-PCR), and the relationship between clinicopathological characteristics and prognosis of glioma patients was analyzed.Results:The relative expressions of OCT4A(2.28±0.85) and OCT4B(2.84±1.29) in 168 glioma tissues were significantly higher than those in normal tissues [(1.05±0.41), (1.18±0.46)] (n t=11.649, 14.798, all n P<0.01), and there was a positive correlation between OCT4A and OCT4B in glioma tissues(n r=0.682, n P=0.001). The expression levels of OCT4A and OCT4B in glioma patients with different pathological stages, pathological types, degree of differentiation and depth of invasion had statistically significant differences(n t=14.695, 11.309, 16.038, 13.721, 17.216, 15.083, 14.871, 15.417, all n P<0.01). Taking the median value of OCT4A and OCT4B mRNA expression as the cutoff value, the patients were divided into low expression group and high expression group.The overall survival of patients with low expression of OCT4A and OCT4B was (24.17±3.41)months and (25.30±4.16)months, which were significantly longer than (15.80±2.93)months and (15.94±3.07)months of high expression patients, the differences were statistically significant(n t=15.639, 16.143, all n P=0.000). Cox regression model analysis showed that the expression levels of OCT4A and OCT4B were independent factors affecting the prognosis of glioma patients.The hazard ratio (95%n CI) was 1.731(0.804-3.181) and 1.605(0.795-2.942), respectively.n Conclusion:The expression levels of OCT4A and OCT4B in glioma tissues are abnormally elevated, which is closely related to the severity of the disease.The patients with high expression of OCT4A and OCT4B have a shorter overall survival period, which has a certain predictive value for the treatment effect and prognosis of glioma.
