目的:观察中医药结合复方蛋氨酸胆碱片防治酒精性肝病的临床疗效,探讨中医药防治酒精性肝病可能的作用机制。方法:治疗组33例采用中医辨证论治加复方蛋氨酸胆碱片治疗,对照组20例单用复方蛋氨酸胆碱片治疗、30例健康人为正常对照,疗程4周。观察临床症状,检测各阶段肝功能、血脂和TNF-α、IL-6水平并作统计分析。结果:治疗组临床症状积分、肝酶、血脂、TNF-α、IL-6水平均明显下降(P<0.01,0.05),其中TC、TNF-α与正常组比较差异无统计学意义(P>0.05)。在改善乏力、黄疸症状、γ-GT、TC方面优于对照组(P<0.01,0.05),治疗组总体疗效优于对照组(P<0.05)。结论:运用中医辨证论治结合复方蛋氨酸胆碱片,能有效降低血清肝功能酶活性,调整肝脏的脂质代谢,影响细胞因子TNF-α、IL-6的表达,改善临床。而抑制细胞因子TNF-α、IL-6等炎性介质的表达,纠正肝脏脂质代谢紊乱,促进酒精在肝内的代谢作用是其可能的作用机制。
Objective: To observe the clinical curative effect of traditional Chinese medicine combined with compound methionine choline in prevention and treatment of alcoholic liver disease and to explore the possible mechanism of Chinese medicine in preventing and treating alcoholic liver disease. Methods: The treatment group (33 cases) was treated with TCM syndrome differentiation and treatment with compound methionine choline. The control group (20 cases) was treated with compound methionine choline and the control group (30 cases) was normal control for 4 weeks. The clinical symptoms were observed and the liver function, blood lipids, TNF-α, IL-6 levels were detected and statistically analyzed. Results: The scores of clinical symptoms, liver enzymes, blood lipids, TNF-α and IL-6 in the treatment group were significantly decreased (P <0.01, 0.05), but there was no significant difference between the treatment group and the normal group (P> 0.05). The improvement of fatigue, jaundice symptoms, γ-GT and TC were better than those of the control group (P <0.01, 0.05). The overall curative effect of the treatment group was better than that of the control group (P <0.05). Conclusion: The TCM syndrome differentiation combined with compound methionine choline tablets can effectively reduce the activity of serum liver enzymes, regulate the lipid metabolism of the liver, and affect the expression of cytokines TNF-α and IL-6 and improve the clinical outcome. But inhibits the expression of cytokines such as TNF-α, IL-6 and other inflammatory mediators, corrects the liver lipid metabolism disorder and promotes the metabolism of alcohol in the liver, which is its possible mechanism.
