目的分析2型糖尿病患者经彩色多普勒超声检查下肢血管相关病变血流在声像图上的临床特点。方法选择336例2型糖尿病合并下肢血管病变患者为观察组,选择同期进行体检的健康人100例作为对照组,均接受彩色多普勒超声检查,对比两组受试者下肢动脉经彩色多普勒超声检查声像图各项指标。结果对照组患者下肢动脉血管内膜光滑程度明显优于观察组,差异具有统计学意义(P<0.01)。观察组患者下肢静脉血管经彩色多普勒超声检查,结果显示血管中膜存在增厚现象,管壁光滑度不足,斑块已经形成,声像图上能够见到血流束边缘呈不规则变化,或出现充盈缺损情况,以及由动脉内存在的硬化斑块导致的血管内狭窄,甚至闭塞,血流束在声像图上逐渐变细,甚至未见任何血流信号,血流频谱显示为单峰或双相波形。结论 2型糖尿病患者利用彩色多普勒超声检查下肢动脉血管内病变,能够直观清晰判断血管内病灶位置及病变发展程度,对早期发病及诊疗下肢血管内病变具有重要的临床意义。
Objective To analyze the clinical features of blood flow in lower extremity vascular related diseases by color Doppler ultrasonography in type 2 diabetic patients. Methods A total of 336 patients with type 2 diabetes mellitus complicated with lower extremity vascular disease were selected as the observation group. 100 healthy subjects undergoing physical examination during the same period were selected as control group. All patients underwent color Doppler ultrasonography. Contrastively, Le son ultrasound sonogram of the indicators. Results In the control group, the intima-smooth degree of lower extremity arteries was significantly better than that of the observation group, the difference was statistically significant (P <0.01). In the observation group, venous blood vessels of the lower extremities were examined by color Doppler ultrasonography. The results showed that there was thickening of the intima of the blood vessel, the smoothness of the wall was insufficient, the plaque was formed, and the edge of the blood stream was irregularly changed on the sonogram , Or filling defects, as well as intravascular stenosis caused by sclerous plaque existing in the artery, or even occlusion, the blood stream becomes narrower on the sonography, even without any blood flow signal, and the blood flow spectrum is shown as Single peak or biphasic waveform. Conclusions Type 2 diabetes mellitus (Doppler echocardiography) can be used to detect intravascular lesions of lower extremities by color Doppler ultrasonography, which can directly and clearly determine the location of intravascular lesions and the degree of lesion development. It is of important clinical significance for early onset and diagnosis of lower extremity intravascular lesions.
