咽旁间隙的良性神经源性肿瘤常与颈部大血管、脑神经、颅底有密切连系,手术死亡率为13~66%,即使在手术成功者中,尚有30%患者可因结扎颈动脉而有严重脑血管紊乱、可致残废。作者们通过对62例(女35,男27;16~67岁)患者的手术经验,探讨了肿瘤的病理组织结构与颈动脉、脑神经的关系,及手术范围等。42例(67%)于术前曾获确诊。术后病理诊断如下:化学感受器瘤29,神经鞘瘤23,神经纤维瘤7,交感神经胚细胞瘤、副神经节瘤和神经节瘤各1根据临床、血管造影、术中发现和组织学检查等,发现21例肿瘤源起自颈动脉体,16例起自迷走神经节或干,10例累及交感神经颈上节及支,
The benign neurogenic tumors of the parapharyngeal space are often closely associated with the large vessels of the neck, brain and skull base. The operative mortality rate is 13 to 66%. Even among those who have achieved successful operation, 30% of patients may be due to ligation. Severe cerebrovascular disorders in the carotid artery can cause disability. The authors explored the relationship between the tumor’s pathological structure, carotid artery, and cranial nerves, and the scope of surgery by using the surgical experience of 62 patients (35 females, 27 males, 16 to 67 years). 42 cases (67%) were diagnosed before surgery. Postoperative pathological diagnosis was as follows: chemoreceptor tumor 29, schwannomas 23, neurofibromas 7, sympathetic neuroblastomas, paraganglioma, and ganglionoma each according to clinical, angiographic, intraoperative findings, and histological examinations. He found that 21 cases of tumors originated from the carotid body, 16 cases from the vagal ganglion or stem, and 10 cases involving the sympathetic upper neck and branches.
