随着超选择性微导管的普及和组织纤维蛋白溶酶原激活剂(t—PA)的应用,使脑栓塞的局部纤溶治疗盛行。但血管再通率仅60%,未能再通的病例预后仍然不良。本文报导一例左大脑中动脉栓塞,经t—PA治疗无效,继之用微型血管成形球囊行经皮腔导管血管成形术(PTA),使血流迅速再通,症状显著改善。 患者60岁,男性。饮酒后突发意识障碍、右偏瘫、失语,入院。查体:嗜睡、失语、右偏瘫、右半身感觉障碍。CT未见异常,发病后1.5小时行血管造影见左侧大脑中动脉M1远端完全闭塞。发病后2.25小时,栓
With the popularity of superselective microcatheter and the use of tissue plasminogen activator (t-PA), local fibrinolytic therapy of cerebral embolism is prevalent. However, the recanalization rate of only 60%, failed to recanalization of the prognosis is still poor. This article reports a case of left middle cerebral artery occlusion, treated with t-PA ineffective, followed by micro-angioplasty balloon percutaneous catheter angioplasty (PTA), the rapid re-flow of blood, the symptoms improved significantly.
