目的 :研究人乳头状瘤病毒 (humanpapillomavirus,HPV) 16和 18型E6蛋白及c-erbB - 2及 p2 1ras基因蛋白的表达与肛癌的关系。方法 :应用免疫组化Envision(二步法 )对肛癌 32例、腺癌 10例及正常肛门粘膜30例HPV16 ,18E6 ,c -erbB - 2及p2 1ras染色。结果 :HPV16、18E6阳性率分别为 78.1% (2 6 /32 ) ,2 0 .0 %(2 /10 ) ,10 .0 % (3/30 ) ,其中鳞癌明显高于正常肛组织的表达 (P <0 .0 1) ,腺癌与正常肛组织表达无明显差异(P >0 .0 5 )。c -erbB - 2及 p2 1ras阳性率分别为 5 9.4% (19/32 ) ,6 0 .0 % (6 /10 ) ,10 .0 % (3/30 )和 5 6 .3% (18/32 ) ,5 0 .0 % (5 /10 )和 3.3% (1/30 ) ,其中鳞癌和腺癌明显高于正常肛组织的表达 (P <0 .0 1)。在鳞癌中 ,E6与c-erbB - 2同时表达 19例 ,占 5 9.4% (19/32 ) ,E6与 p2 1ras同时表达 18例 ,占 5 6 .3% (18/32 ) ,与腺癌及正常肛粘膜组织中二者同时表达的病例相比有明显差异 (P <0 .0 1)。结论 :HPV16 ,18E6 ,c -erbB - 2及p2 1ras与肛鳞癌的发生发展有一定相关性。c -erbB - 2及p2 1ras与肛鳞癌、腺癌的发生发展均有相关性。HPV16 ,18E6 ,c-erbB - 2及p2 1ras共同作用可能是肛癌发生发展的重要因素
Objective: To investigate the relationship between human papillomavirus (HPV) 16 and 18 E6 protein, c-erbB-2 and p2 1ras gene protein expression and anal cancer. METHODS: Envision (two-step) immunohistochemistry was used to stain 32 cases of anal cancer, 10 cases of adenocarcinoma, and 30 cases of normal anal mucosa. HPV16, 18E6, c-erbB-2 and p2 1ras staining. Results: The positive rates of HPV16 and 18E6 were 78.1% (2 6 /32), 2 0.0% (2 /10), and 10.0% (3/30) respectively. Among them, squamous cell carcinoma was significantly higher than that of normal anal tissue. (P <0. 0 1), there was no significant difference in the expression of adenocarcinoma and normal anal tissue (P > 0.05). The positive rates of c-erbB-2 and p2 1ras were 5 9.4% (19/32), 6 0.0% (6/10), 10.0% (3/30), and 56.3% (18/ 32), 50% (5/10) and 3.3% (1/30), of which squamous cell carcinoma and adenocarcinoma were significantly higher than those of normal anal tissue (P < 0.01). In squamous cell carcinoma, E6 and c-erbB-2 were simultaneously expressed in 19 cases, accounting for 5 9.4% (19/32). E6 and p2 1ras were simultaneously expressed in 18 cases, accounting for 56.3% (18/32), and in glands. There was a significant difference in the cases of simultaneous expression of cancer and normal anal mucosa (P < 0.01). Conclusion : HPV16, 18E6, c-erbB-2 and p2 1ras are related to the occurrence and development of anal squamous cell carcinoma. Both c-erbB-2 and p2 1ras were associated with the development of anal squamous cell carcinoma and adenocarcinoma. The combined effect of HPV16, 18E6, c-erbB-2 and p2 1ras may be an important factor in the development of anal cancer
