目的:探讨外周血单核细胞磷酸肌醇蛋白聚糖3(GPC-3)mRNA联合血浆γ-谷氨酰转肽酶(GGT)检测诊断甲胎蛋白(AFP)阴性原发性肝细胞肝癌(HCC)的价值。方法:采用RT-PCR方法分别检测39例AFP阴性HCC患者、39例非HCC肝病患者及40例健康自愿者外周血单核细胞GPC-3 mRNA的表达;用全自动生化分析仪检测39例AFP阴性HCC患者血浆GGT水平(>58 U/L为阳性),分析两者单独和联合检测阳性率的差异。结果:39例HCC患者外周血单核细胞GPC-3 mRNA表达检出率为62%(24/39),而在非HCC的肝病患者及健康对照者外周血单核细胞中GPC-3 mRNA表达均为阴性,特异度达100%;39例HCC患者血浆GGT升高阳性率80%(31/39)。GPC-3 mRNA联合GGT检测的阳性率95%,明显高于两者单独检测的阳性率(均P<0.05)。结论:AFP阴性HCC患者外周血单核细胞中GPC-3 mRNA有较高的特异性表达,GPC-3 mRNA联合GGT检测有助于AFP阴性的HCC早期发现。
OBJECTIVE: To investigate the diagnostic value of peripheral blood mononuclear cell glypican 3 (GPC-3) mRNA and gamma-glutamyl transpeptidase (GGT) in the diagnosis of AFP negative hepatocellular carcinoma HCC) value. Methods: The expression of GPC-3 mRNA in peripheral blood mononuclear cells of 39 AFP-negative HCC patients, 39 non-HCC liver disease patients and 40 healthy volunteers were detected by RT-PCR. The expression of 39 AFP Negative HCC patients with plasma GGT levels (> 58 U / L was positive), the two were analyzed separately and combined detection of the positive rate differences. Results: The expression of GPC-3 mRNA in peripheral blood mononuclear cells of 39 HCC patients was 62% (24/39), while the expression of GPC-3 mRNA in peripheral blood mononuclear cells of non-HCC patients and healthy controls All were negative and the specificity was 100%. The positive rate of plasma GGT in 39 HCC patients was 80% (31/39). The positive rate of GPC-3 mRNA combined with GGT was 95%, which was significantly higher than the positive rate of both (all P <0.05). CONCLUSION: GPC-3 mRNA is highly expressed in peripheral blood mononuclear cells of AFP-negative HCC patients. The detection of GPC-3 mRNA combined with GGT is helpful for the early detection of AFP-negative HCC.
