目的探讨健康老年人肾脏CT灌注参数的变化。方法28例正常人按年龄分三组:Ⅰ组41~岁,Ⅱ组51~岁,Ⅲ组61~岁,全部采用Siemens 16层螺旋CT行双侧肾脏灌注扫描。采集数据,经后处理,获得肾脏血流灌注图像和时间?密度曲线(TDC),通过感兴趣区获得皮髓质的不同灌注参数,包括肾血流量(BF)、肾血容量(BV)、肾脏对比剂平均通过时间(MTT)和肾脏强化高峰时间(TTP)。结果61~岁组肾皮质的BF值较41~岁组及51~岁组明显减低,差异有统计学意义(P<0.05);41~岁组及51~岁组之间BF值差异无统计学意义(P>0.05);MTT值随年龄增大,有增加趋势,但差异均无统计学意义(P>0.05)。肾髓质灌注参数三组间比较,差异均无统计学意义(P>0.05),但BV和MTT值有增加趋势。结论CT灌注成像图更直观形象地显示了肾脏皮、髓质的血流灌注状况;年龄老化对肾脏血流的影响主要是皮质;为临床了解老化肾脏病理生理变化提供量化依据。
Objective To investigate the changes of renal CT perfusion parameters in healthy elderly. Methods 28 normal subjects were divided into three groups by age: group Ⅰ 41 to age group Ⅱ 51 to group year Ⅲ and group 61 to group year old. All patients were diagnosed by Siemens 16 - slice spiral CT with bilateral renal perfusion scan. Data were collected and post-processed to obtain renal perfusion image and time density curve (TDC). Different perfusion parameters of corticomedullary area were obtained through the region of interest, including renal blood flow (BF), renal blood volume (BV) Renal Contrast Agent Mean Time To Time (MTT) and Renal Intensive Rhythm (TTP). Results The BF value of renal cortex in 61 ~ age group was significantly lower than that in 41 ~ 51 and 51 ~ age groups (P <0.05). There was no statistical difference in BF value between 41 ~ (P> 0.05). MTT value increased with age, but the difference was not statistically significant (P> 0.05). There was no significant difference between the three groups in medullary perfusion parameters (P> 0.05), but BV and MTT values tended to increase. Conclusion CT perfusion imaging more directly shows the renal cortex and medullary blood perfusion; aging of the renal blood flow is mainly cortex; provide a quantitative basis for understanding the pathological and physiological changes of aging kidneys.
