目的评价川芎嗪涂层肾动脉支架(TES)对肾动脉内膜增生的短期影响。方法小猪10只均分为TES组(单侧肾动脉内植入TES)和BMS组(植入金属裸支架)。支架植入前后及35d后行定量肾动脉造影(QRA)及血管内超声(IVUS),观察肾动脉晚期管腔丢失及内膜增生情况。术后第35天处死动物,检测支架段血管内膜增生厚度,免疫组化测定增殖细胞核抗原(PCNA),扫描电镜观察内皮覆盖情况。结果两组晚期管腔丢失、平均面积狭窄百分比等均无显著性差异。两组支架内面积和管腔内面积相近,TES组新生内膜面积和内膜PCNA阳性率均较BMS组略有减少(P>0.05)。扫描电镜显示,TES组35d后内皮细胞覆盖完全,BMS组覆盖基本完整。结论 TES具有抑制内膜增殖的倾向,且不影响血管内皮愈合。
Objective To evaluate the short-term effect of ligustrazine-coated renal artery stent (TES) on intimal hyperplasia of renal artery. Methods Ten piglets were randomly divided into TES group (unilateral renal artery implanted TES) and BMS group (bare metal stent). Quantitative renal artery angiography (QRA) and intravascular ultrasound (IVUS) were performed before and after stenting and 35 days after implantation. Late luminal loss and intimal hyperplasia of renal artery were observed. Animals were sacrificed on the 35th day after operation. The intimal hyperplasia thickness of the stent segment was detected. The proliferating cell nuclear antigen (PCNA) was detected by immunohistochemistry. The endothelial coverage was observed by scanning electron microscopy. Results There was no significant difference between the two groups in late luminal loss and the mean percentage of stenosis. The area of the stent and the area of the lumen were similar in both groups. The neointimal area and the intimal PCNA positive rate in the TES group were slightly decreased compared with those in the BMS group (P> 0.05). Scanning electron microscopy showed that the endothelial cells were completely covered in the TES group 35 days after transplantation, and the coverage in the BMS group was basically intact. Conclusion TES has a tendency to inhibit the proliferation of the intima, and does not affect the vascular endothelial healing.
