目的:检测67例胆囊癌、10例胆囊腺瘤及癌旁粘膜的p53蛋白表达,分析其与肿瘤组织学类型的关系。方法:应用免疫组化技术。结果:p53蛋白胆囊癌阳性表达为59.7%,腺瘤为20%。阳性细胞主要分布在腺瘤的异型增生区;癌旁粘膜p53蛋白阳性表达在异型增生上皮2/4例,单纯增生上皮5/14例,肠化上皮1/2例。腺癌p53蛋白阳性率Ⅰ级48.48%,Ⅱ级90.9%,Ⅲ级71.42%,差异有显著性(P<0.05);Ⅰ级腺癌中,乳头状腺癌阳性率高达66.66%。结论:腺瘤及癌旁粘膜中的异型增生、化生及单纯性增生上皮p53蛋白过度表达,均有发生恶变的潜在可能。
OBJECTIVE: To detect the expression of p53 protein in 67 cases of gallbladder carcinoma, 10 cases of gallbladder adenoma and paraneoplastic mucosa, and to analyze the relationship between the expression of p53 protein and histological types. Methods: The application of immunohistochemistry. Results: The positive expression of p53 gallbladder carcinoma was 59.7%, and adenoma was 20%. The positive cells were mainly distributed in the dysplasia of adenomas; the para-cancer mucosa p53 protein was positively expressed in 2/4 dysplasia epithelium, 5/14 simple hyperplasia epithelium, and 1/2 intestinal metaplasia. The positive rate of p53 protein in adenocarcinoma was 48.48% in grade I, 90.9% in grade II, and 71.42% in grade III. The difference was statistically significant (P<0.05). Papillary adenocarcinoma was positive in grade I adenocarcinoma. The rate is as high as 66.66%. CONCLUSIONS: The overexpression of p53 protein in dysplasia, metaplasia, and simple hyperplasia of adenomas and paraneoplastic mucosa all have potential for malignant transformation.
