目的观察急性脑梗死(ACI)早期辛伐他汀治疗对血脂和炎症反应标志物C!反应蛋白(CRP)的影响及差异,探讨他汀类药物在急性脑梗死早期治疗中的作用。方法选择急性脑梗死住院患者96例,随机分为辛伐他汀治疗组和非辛伐他汀治疗组,比较入院前和出院后甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL!ch)、高密度脂蛋白胆固醇(HDL!ch)和CRP水平。同期选取15例健康者作为对照组。结果①出院时辛伐他汀组与非辛伐他汀组的血浆CRP为(9.32±6.40)mg/L和(12.22±7.18)mg/L,显著低于入院时的(28.12±25.62)mg/L和(27.98±27.01)mg/L(P<0.01);出院时辛伐他汀组血浆CRP亦显著低于非辛伐他汀组(P<0.01)。②据入院时血浆CRP水平,将急性脑梗死患者分为CRP高值组和低值组,出院与入院时比较,高值组和低值组血浆CRP均显著降低(P<0.01),但只有高值组出院时辛伐他汀治疗者血浆CRP显著低于非辛伐他汀治疗者(P<0.05)。结论急性脑梗死患者早期辛伐他汀治疗使CRP显著降低,有明显抗炎症作用。
Objective To observe the effects of early simvastatin on blood lipids and C reactive protein (CRP) in patients with acute cerebral infarction (ACI) and to investigate the role of statins in the early treatment of acute cerebral infarction. Methods 96 hospitalized patients with acute cerebral infarction were randomly divided into simvastatin treatment group and non-simvastatin treatment group. The levels of triglyceride (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL! Ch), high density lipoprotein cholesterol (HDL! Ch) and CRP levels. In the same period, 15 healthy people were selected as the control group. Results The plasma levels of CRP in the simvastatin group and the non-simvastatin group were (9.32 ± 6.40) mg / L and (12.22 ± 7.18) mg / L respectively at the time of discharge, which were significantly lower than those at admission (28.12 ± 25.62) mg / L And (27.98 ± 27.01) mg / L respectively (P <0.01). The plasma CRP in simvastatin group was also significantly lower than that in non-simvastatin group at discharge (P <0.01). ② According to the level of plasma CRP on admission, patients with acute cerebral infarction were divided into high CRP group and low CRP group. The plasma CRP in high-value group and low-value group were significantly lower at discharge and admission (P <0.01) Plasma CRP was significantly lower in simvastatin treatment group than in non-simvastatin treatment group (P <0.05). Conclusions Early simvastatin treatment in patients with acute cerebral infarction significantly reduces CRP and has obvious anti-inflammatory effects.
