目的 :研究微种植支抗在Tweed-Merrifield定向力矫治技术中的应用,对治疗效果的影响。方法 :回顾性分析2009~2014年就诊于南京医科大学附属口腔医院的18位诊断为安氏Ⅱ类一分类或者安氏Ⅰ类的需要拔除上下颌4个双尖牙的上颌前突成年患者,这些患者愿意选择植入微种植体支抗,并使用Tweed-Merrifield定向力矫治技术进行矫治。分别对治疗前后的多项数据进行测量对比,研究其意义。结果:18位患者均顺利完成矫治,面型均得到较大改善。治疗前后的配对t检验显示下中切牙-下颌平面角(IMPA)平均减少了7.99°(P<0.01),ANB角平均减少了1.56°(P<0.01),OP角平均减少了0.54°(P<0.05),Z角平均增大了11.42(P<0.01)。结论:在Tweed-Merrifield定向力矫治技术中,微种植体能够提供足够的支抗,并取得较为满意的治疗效果。由于其不依赖患者的配合,有更广的适用范围。
OBJECTIVE: To study the application of micro-implanting support in the Tweed-Merrifield directed force correction technique and its effect on the therapeutic effect. Methods: 18 adult patients with maxillary protrusion who were diagnosed as Class Ⅱ division Ⅰ or Class Ⅰ requiring extraction of maxillary and mandibular quadrant from 2009 to 2014 at the Affiliated Stomatological Hospital of Nanjing Medical University were retrospectively analyzed. These patients are willing to choose implant micro-implant support, and use Tweed-Merrifield directional correction technology for orthodontic treatment. A number of data before and after treatment were measured and compared to study the significance. Results: All 18 patients successfully completed the treatment, and the facial features were greatly improved. Paired t-tests before and after treatment showed an average decrease of IMPA of 7.99 ° (P <0.01), an ANB of 1.56 ° (P <0.01) and an average reduction of OP of 0.54 ° (P <0.01) P <0.05), Z angle increased by an average of 11.42 (P <0.01). Conclusion: In the Tweed-Merrifield directed force correction technique, micro-implants can provide adequate support and achieve satisfactory results. Because it does not rely on the cooperation of patients, there is a wider scope of application.
