目的探讨脐血Hb Bart’s定量筛查α-地中海贫血(简称α-地贫)的价值及α-地贫新生儿脐血红细胞参数的变化。方法用Bayer ADVIA120全自动血细胞分析仪检测新生儿脐血红细胞数参数,Sebia全自动电泳仪对脐血进行Hb Bart’s定量检测,比较Hb Bart’s阳性组与阴性组脐血红细胞参数的差异。结果在1219份脐血样本中共有36份检出HbBart’s,其中静止型12例,标准型23例,中间型1例,未检出重型α-地贫,总检出率为2.95%。Hb Bart’s阳性组新生儿脐血RBC显著高于Hb Bart’s阴性组(P﹤0.01),而HB、MCV、MCH、HCT均显著低于Hb Bart’s阴性组(P﹤0.01)。结论脐血Hb Bart’s定量检测可有效筛查出新生儿α-地贫,脐血红细胞数参数可作为α-地贫的筛查指标。
Objective To investigate the value of quantitative screening of α-thalassemia (HbAb) in umbilical cord blood and the changes of umbilical cord blood erythrocyte parameters in α-thalassemia neonates. Methods Bayer ADVIA120 automatic hematology analyzer was used to detect neonatal umbilical cord blood erythrocyte parameters. Hb Bart’s quantification was performed on Sebia automated electrophoresis apparatus to compare cord blood red blood cell parameters between Hb Bart’s positive group and negative group. Results A total of 36 HbBart’s were detected in 1219 samples of umbilical cord blood, of which 12 were in resting type, 23 in standard type and 1 in intermediate type. The detection rate of heavy alpha-thalassemia was 2.95%. Hb Bart’s positive group neonatal cord blood RBC was significantly higher than Hb Bart’s negative group (P <0.01), while HB, MCV, MCH, HCT were significantly lower than Hb Bart’s negative group (P <0.01). Conclusion Quantitative detection of Hb Bart’s in umbilical cord blood can effectively screen neonatal α-thalassemia, and the number of cord blood red blood cells can be used as a screening index of α-thalassemia.
