目的:以“五全”(即全人、全家、全护、全面、全程简称五全)社区护理服务模式开展居家护老服务,为迫切需要帮助的老年人提供生活照料、健康维护等服务,探索一条可持续发展的、符合中国国情的居家护老服务模式。方法:按楼房的单元划分,每个单元建立保健责任人、楼长及社区责任人各一位,保健责任人提供医疗保健转诊服务,楼长为老人提供生活照料服务,社区责任人提供政策帮扶。结果:通过开展居家护老“五全”社区护理服务模式,为护理服务延伸到社区搭建了平台,满足了居家老人的医疗保健需求,推动社区护理服务模式的改进,提高服务质量,促进社区护理服务队伍不断发展壮大。结论:居家护老“五全”社区护理服务模式,满足了居民生活和医疗服务需求,为社区规范化建设和“多元参与共建”社区治理奠定了坚实的基础。
OBJECTIVE: To provide home-based care services in the form of community care services with “All Five” (that is, whole-person, whole-family, full care, comprehensive and omnibearing), providing life care and health maintenance for the elderly in urgent need of help Services, explore a sustainable development, in line with China’s national home care services model. Methods: According to the units of buildings, each unit established one health care director, one director and one community responsibility person, the health care workers provided the health care referral service, the director provided the life care service for the elderly and the community responsible person provided the policy help. Results: Through the development of home care services and “Five-State” community care service model, a platform for nursing services extended to the community was established to meet the needs of the elderly in home care, promote the improvement of community nursing service mode, improve service quality and promote Community care service team continue to grow and develop. Conclusion: The “Five Fullness” community care service model of home nursing care meets the needs of residents’ lives and medical services, laying a solid foundation for community standardization and community governance of “multiple participation and co-construction.”
