目的 回顾性分析Ⅰ期非霍奇金淋巴瘤 (NHL)的治疗结果和影响预后的因素。方法 Ⅰ期NHL 187例 ,男性 12 2例 ,女性 6 5例。原发于淋巴结者 75例 ,112例原发于淋巴结外器官。按照工作分类方法 (WorkingFormulation)进行分类 ,低度恶性 7例 ,中度恶性 82例 ,高度恶性 82例 ,未能分类 16例。单纯放射治疗 84例 ,放射治疗 +化疗 38例 ,化疗 +放射治疗 +化疗 40例 ,化疗 +放射治疗 16例和单纯化疗 9例。放射治疗采用局部野或局部扩大野照射 ,原发部位剂量为DT5 0～5 5Gy ,邻近淋巴结区预防剂量DT45Gy。化疗以CHOP和BACOP方案为主 ,多为 4～ 6个周期。应用Kaplan Meier方法进行生存分析 ,Log rank方法进行比较分析。 结果 全部病例 5年生存率为83.0 %。病理低、中、高度恶性的 5年生存率分别为 10 0 .0 % ,82 .3%和 83.0 % ,P =0 .77。原发淋巴结和结外器官的 5年生存率为 79.5 %和 85 .1% ,P =0 .33。单纯放射治疗 ,放射治疗 +化疗 ,化疗 +放射治疗 +化疗 ,化疗 +放射治疗和单纯化疗的 5年生存率分别为 83.8% ,88.5 % ,82 .1% ,71.4% ,77.8%。P =0 .6 2。年龄≤ 5 0岁和 >5 0岁的治疗后 5年生存率分别为 86 .2 %和 78.7% ,P=0 .18。男性和女性的 5年生存率分别为 79.1%和 90 .3% ,P <0 .0 5。 75例原发淋巴
Objectives To retrospectively analyze the treatment outcome and prognostic factors of stage I non-Hodgkin’s lymphoma (NHL). Methods There were 187 cases of stage I NHL. There were 122 males and 65 females. Of the 75 patients with primary lymph nodes, 112 were primary organs outside the lymph nodes. According to the Working Formulation classification, there were 7 cases of low grade malignancy, 82 cases of moderate malignancy, 82 cases of high grade malignancy, and 16 cases of failure to classify. There were 84 cases of radiotherapy alone, 38 cases of radiotherapy + chemotherapy, 40 cases of chemotherapy + radiation therapy + chemotherapy, 16 cases of chemotherapy + radiation therapy and 9 cases of chemotherapy alone. Radiotherapy was performed with local or local augmented field irradiation at a dose of DT50-55Gy at the primary site and DT45Gy at the adjacent lymph node. Chemotherapy is mainly based on CHOP and BACOP regimens, with 4 to 6 cycles. Kaplan Meier method was used for survival analysis and Log rank method was used for comparative analysis. Results The 5-year survival rate was 83.0% in all cases. The 5-year survival rates of low-, medium-, and high-grade malignancies were 100.0%, 82.3%, and 83.0%, respectively, P = 0.77. The 5-year survival rate of primary lymph nodes and extranodal organs was 79.5 % and 85.1%, P = 0.33. The 5-year survival rates of radiotherapy alone, radiation therapy + chemotherapy, chemotherapy + radiation therapy + chemotherapy, chemotherapy + radiation therapy and chemotherapy alone were 83.8%, 88.5 %, 82.1%, 71.4%, and 77.8%, respectively.
