目的探讨天津市汉族老年人群内皮型一氧化氮合酶(eNOS)基因G894T多态性与颈动脉粥样硬化(CAS)的关联性。方法选择接受颈动脉超声检查患者427例,根据超声检查结果分为CAS组1 30例和对照组297例。采用PCR-RFLP方法分析G894T多态性基因型,同时对所有对象检验血脂等危险因素。结果 CAS组基因型GT、GT+TT和T等位基因频率明显高于对照组(P<0.05,P<0.01)。调整了年龄和性别后,基因型GT+TT与CAS的关联差异有统计学意义(OR=1.89,95%CI:1.20~2.98,P=0.007)。对其他危险因素调整后,logistic回归分析,基因型GT+TT不是CAS的独立危险因素(OR=1.43,95%CI:0.85~2.40,P=0.1 78)。在CAS组中,不同斑块类型的分布、斑块总面积和Crouse积分在T等位基因携带者与非携带者差异无统计学意义(P>0.05)。结论 eNOS基因G894T多态性的基因型GT+TT与CAS的发生相关,但不是CAS的独立危险因素,与CAS的严重程度亦无关联。
Objective To investigate the association between G894T polymorphism of endothelial nitric oxide synthase (eNOS) gene and carotid atherosclerosis (CAS) in elderly Han population in Tianjin. Methods 427 patients undergoing carotid ultrasonography were selected. According to the results of ultrasound examination, there were 130 cases in CAS group and 297 cases in control group. The genotypes of G894T polymorphism were analyzed by PCR-RFLP and risk factors such as lipids were tested for all subjects. Results The frequencies of GT, GT + TT and T allele in CAS group were significantly higher than those in control group (P <0.05, P <0.01). After adjusting for age and gender, the association of genotypes GT + TT with CAS was statistically significant (OR = 1.89, 95% CI: 1.20-2.98, P = 0.007). After adjustment for other risk factors, genotype GT + TT was not an independent risk factor for CAS by logistic regression analysis (OR = 1.43, 95% CI: 0.85-2.40, P = 0.178). In the CAS group, the distribution of different plaque types, total plaque area and Crouse score were not significantly different between carriers and non carriers of T allele (P> 0.05). Conclusion The genotype GT + TT of G894T polymorphism of eNOS gene is related to the occurrence of CAS, but not an independent risk factor for CAS, nor is it related to the severity of CAS.
