目的 :探讨鼻内窥镜下非侵袭性上颌窦曲菌病的治疗及影响疗效的因素。方法 :分析我院行功能性鼻内窥镜手术 (Functional Endoscopic Sinus Surgery,FESS)治疗的非侵袭性上颌窦曲菌病 41例的手术径路及抗真菌剂的使用对疗效及转归的影响。结果 :双径路 (经尖牙窝加上颌窦自然开口 )术式的术后愈合期为 4.7周 ,而单径路 (经上颌窦自然开口 )术式的术后愈合期为 9.3周 ,两组差异有显著性意义 (P <0 .0 1)。使用抗真菌剂冲洗术腔组的术后愈合期为 6 .4周 ,未使用抗真菌剂组的术后愈合期为 6 .7周 ,但其差异无统计学意义 (P >0 .0 5 )。结论 :FESS是治疗非侵袭性上颌窦曲菌病的有效方法 ,双径路疗效优于单径路 ,是否用抗真菌剂冲洗术腔对疗效无明显影响。
Objective: To explore the treatment of non-invasive maxillary sinus aspergillosis under nasal endoscopy and the factors influencing the curative effect. Methods: The effects of surgical approach and antimycotic on 41 patients with noninvasive maxillary sinus aspergillosis undergoing Functional Endoscopic Sinus Surgery (FESS) in our hospital were analyzed. Results: The diameter of the double path (maxillary sinus plus maxillary natural opening) was 4.7 weeks and the healing time of single path (maxillary sinus natural opening) was 9.3 weeks. There was no significant difference between the two groups There was significant (P <0. 01). The healing time of the group receiving antifungal agent was 6.4 weeks, while that of the non-use antifungal group was 6.7 weeks, but the difference was not statistically significant (P> 0.05). ). Conclusion: FESS is an effective method for the treatment of non-invasive maxillary sinus aspergillosis. The double-track approach is superior to single-track approach. Whether to use antifungal agent to flush the surgical cavity has no significant effect on the curative effect.
