目的了解三明市尘肺病患者肺功能与血气分析的基本情况,为尘肺病患者工伤保险待遇提供科学依据。方法对160例尘肺病患者进行肺功能检测和血气分析,分析两者同尘肺病期别、工龄、年龄、致残等级的关系,以及肺功能与血气分析的关系。结果随着尘肺病期别提高,PO_2、FVC和FEV_(1.0)各指标有降低趋势(rs=-0.572、-0.644、-0.717,P<0.05),随着致残等级加重,三者也有降低趋势(r_s=-0.560、-0.443、-0.458,P<0.05);血气分析指标PO_2与肺功能FVC、FEV1.0之间均呈正相关关系(r_s=0.768、0.823,P<0.05)。分别采用血气分析和肺功能检测结果来表达尘肺病患者损伤程度,两方法所得结果差异无统计学意义(χ~2=4.950,P>0.05);分别用血气分析和肺功能检测鉴定尘肺病患者致残等级,两种方法所得结果差异无统计学意义(χ~2=0.180,P>0.05)。结论血气分析在尘肺病鉴别、致残等级鉴定中客观、直接,建议采用血气分析PO2检查作为首选指标,肺功能FVC和FEV1.0作为辅助指标鉴定尘肺病患者致残等级。
Objective To understand the basic situation of pulmonary function and blood gas analysis in patients with pneumoconiosis in Sanming City and provide a scientific basis for the treatment of industrial injury insurance for patients with pneumoconiosis. Methods Totally 160 patients with pneumoconiosis were tested for pulmonary function and blood gas analysis. The relationship between pneumoconiosis and pneumoconiosis stage, length of service, age and disability level was analyzed. The relationship between lung function and blood gas analysis was analyzed. Results With the increase of pneumoconiosis stage, the indexes of PO_2, FVC and FEV_ (1.0) decreased (rs = -0.572, -0.644, -0.717, P <0.05) (R_s = -0.560, -0.443, -0.458, P <0.05). There was a positive correlation between blood gas analysis index PO_2 and pulmonary function FVC and FEV1.0 (r_s = 0.768,0.823, P <0.05). The blood gas analysis and pulmonary function test results were used to express the degree of injury in patients with pneumoconiosis. There was no significant difference between the two methods (χ ~ 2 = 4.950, P> 0.05).
