目的:分析评价注射用盐酸头孢吡肟连续静滴与注射用盐酸头孢吡肟-头孢丙烯胶囊序贯给药治疗中、重度社区获得性肺炎的经济学效果。方法:将60例中、重度社区获得性肺炎患者随机分为A、B组,每组各30例,其中A组采用注射用盐酸头孢吡肟2 g+生理盐水100 mL连续静滴,bid,疗程10 d;B组注射用盐酸头孢吡肟2 g+生理盐水100mL静脉滴注,3~5 d,病情好转后给予头孢丙烯胶囊0.5 g,bid,po,总疗程10 d。观察两组的临床疗效、细菌清除率及不良反应等,并对2种治疗方案进行经济学效果评价分析。结果:两组的有效率分别为93.33%和96.67%,细菌清除率为93.33%和87.5%,无明显不良反应,两组比较无显著性差异(P>0.05)。但B组的平均治疗总费用及抗生素费用分别为1 541.39元和1 200.89元,明显低于A组3 225.80元和2 589.60元,两组比较有显著性差异(P<0.05)。结论:对于中、重度社区获得性肺炎患者,采用序贯给药治疗不但安全、有效,而且更加经济、合理。
OBJECTIVE: To evaluate and evaluate the economic effect of continuous intravenous infusion of cefepime hydrochloride for injection and sequential administration of cefepime-cefprozil hydrochloride capsules for the treatment of moderate and severe community-acquired pneumonia. Methods: Sixty patients with moderate or severe community-acquired pneumonia were randomly divided into group A and group B, with 30 cases in each group. Group A received intravenous injection of cefepime hydrochloride 2 g + 10 d. The patients in group B were injected intravenously with 2 mL of cefepime hydrochloride and 100 mL normal saline for 3 to 5 days. The patients in group B received cefprozil 0.5 g once daily for 10 days after treatment. The total course of treatment was 10 days. The clinical efficacy, bacterial clearance and adverse reactions of the two groups were observed, and the economic effects of the two treatment regimens were evaluated. Results: The effective rates of the two groups were 93.33% and 96.67% respectively. The bacterial clearance rate was 93.33% and 87.5%. There was no obvious adverse reaction between the two groups. There was no significant difference between the two groups (P> 0.05). However, the average cost of treatment and antibiotics in group B were respectively 1 541.39 yuan and 1 200.89 yuan, which was significantly lower than that of group A 3 225.80 yuan and 2 589.60 yuan. There was significant difference between the two groups (P 0.05). Conclusion: For patients with moderate or severe community-acquired pneumonia, sequential administration is not only safe and effective, but also more economical and reasonable.
