目的探讨高频二维超声和彩色多普勒成像技术在乳腺良恶性肿瘤诊断中的图像特点。方法选择2012年7月-2015年6月上虞人民医院经病理诊断为乳腺恶性肿瘤102例作为恶性组,乳腺良性肿瘤110例作为良性组。高频二维超声扫查肿瘤的形态、大小、边界、内部回声、包膜、钙化、淋巴结情况;彩色多普勒成像技术探查血流信号及频谱。结果乳腺恶性肿瘤组边界不清楚、形态不规则、钙化、内部回声不均匀、有纵横比、有后方衰减、有淋巴结、有淋巴结肿大所占比例均高于乳腺良性肿瘤组(P<0.05)。乳腺良性肿瘤和恶性肿瘤的彩色多普勒超声血流分级情况比较差异有统计学意义(P<0.05)。乳腺恶性肿瘤的血管阻力指数(RI)、颈动脉舒张末期血流速度(EDV)和颈动脉收缩期最大流速(PSV)值均高于良性肿瘤组(P<0.05)。乳腺恶性肿瘤组BIRADS分级4级+5级所占比例明显高于良性肿瘤组(P<0.05)。结论高频二维超声和彩色多普勒成像特点在乳腺良性肿瘤和恶性肿瘤的鉴别诊断中具有重要价值。
Objective To investigate the features of high frequency two-dimensional ultrasound and color Doppler imaging in the diagnosis of benign and malignant breast tumors. Methods Select July 2012 - June 2015 Shangyu People’s Hospital pathologically diagnosed as breast cancer in 102 cases as a malignant group, 110 cases of benign breast tumors as a benign group. The morphology, size, boundary, internal echo, capsule, calcification and lymph node status were examined by two-dimensional high-frequency ultrasound. The color Doppler imaging was used to probe the blood flow signal and the spectrum. Results The borderline of breast malignant tumor group was unclear, irregular shape, calcification, internal echo unevenness, aspect ratio, posterior attenuation, lymph node, lymph node enlargement were all higher than those in benign breast tumor group (P <0.05) . The color Doppler flow grade of breast benign and malignant tumors was significantly different (P <0.05). Vascular resistance index (RI), carotid end-diastolic velocity (EDV) and carotid artery systolic velocity (PSV) in breast cancer were significantly higher than those in benign tumor (P <0.05). The proportion of BIRADS grade 4 and grade 5 in breast cancer group was significantly higher than that in benign tumor group (P <0.05). Conclusion High-frequency two-dimensional ultrasound and color Doppler imaging features in the differential diagnosis of benign and malignant breast tumors have an important value.
