目的探讨农村地区中老年人群的腰围身高比值(waist to height ratio,WHtR)与血压、血脂的关系。方法采用统一编制调查表记录2013年在4个县共4 281人的血压、血脂、WHtR等指标。WHtR对血压、血脂具体指标影响采用主成分回归分析方法。结果农村地区人群WHtR≥0.5组高血压患病率比WHtR<0.5组高20.13%(95%CI:17.33~22.38),WHtR≥0.5组血脂异常患病率比WHtR<0.5组高16.89%(95%CI:14.20~19.58),WHtR≥0.5组高胆固醇血症、高低密度脂蛋白胆固醇血症、低高密度脂蛋白胆固醇血症、高甘油三酯血症患病率均比WHtR<0.5组高;主成分回归分析表明,WHtR与其他危险因素相比对收缩压(systolic blood pressure,SBP),舒张压(diastolic blood pressure,DBP),总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、高密度脂蛋白胆固醇(high density lipoprotein-cholesterol,HDL-C),低密度脂蛋白胆固醇(low density lipoprotein-cholesterol,LDL-C)均有较大影响,标准回归系数bi分别为0.24、0.14、0.06、0.08、0.07、-0.10。WHtR≥0.5与WHtR<0.5相比SBP的值大0.24个单位,即WHtR≥0.5与WHtR<0.5相比DBP、TC、TG、LDL-C值分别大0.14、0.06、0.08、0.07个单位,WHtR≥0.5与WHtR<0.5相比HDL-C的值小0.10个单位。结论农村地区人群WHtR与血压、血脂各指标关系密切,可以通过降低WHtR改善血压、血脂各项指标。
Objective To investigate the relationship between waist to height ratio (WHtR) and blood pressure and blood lipid in middle-aged and elderly people in rural areas. Methods A unified questionnaire was used to record the blood pressure, blood lipid, WHtR and other indexes of 4 281 people in 4 counties in 2013. WHtR on blood pressure, blood lipids specific indicators using principal component regression analysis. Results The prevalence of hypertension in WHtR≥0.5 group was 20.13% (95% CI: 17.33 ~ 22.38) in WHtR≥0.5 group and 16.89% (95% CI: 95% CI) in WHtR≥0.5 group % CI: 14.20 ~ 19.58). The prevalence of hypercholesterolemia, high and low density lipoprotein cholesterol, low density lipoprotein cholesterol and hypertriglyceridemia in WHtR≥0.5 group were significantly lower than those in WHtR <0.5 group The regression analysis of principal components showed that WHtR had no significant effect on SBP, diastolic blood pressure (DBP), total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C) and low density lipoprotein-cholesterol (LDL-C), the standard regression coefficients bi were 0.24 , 0.14,0.06,0.08,0.07, -0.10. WHtR≥0.5 Compared with WHtR <0.5, the value of SBP was 0.24 units, that is, WHtR≥0.5 and WHtR <0.5, the values of DBP, TC, TG and LDL-C were 0.14,0.06,0.08 and 0.07 respectively, WHtR The value of HDL-C is 0.10 unit smaller than 0.5 for WHtR <0.5. Conclusion The WHtR in rural areas is closely related to the indexes of blood pressure and blood lipid, and the indexes of blood pressure and blood lipid can be improved by reducing the WHtR.
