目的探讨健康成人髌软骨磁共振弥散张量成像(DTI)FA值变化规律。方法选择健康志愿者40名80个膝关节,采用GE Discovery MRI 750 3.0T磁共振扫描仪DTI成像,将数据传输到GE工作站(ADW4.6),经Functool中软件处理后产生髌软骨的FA伪彩图,选取股骨内侧髁、外侧髁、股骨髌面3个部位为测量感兴趣区,测量软骨各部位表层、中间层、深层的FA值,每点重复测量3次,取平均值,由3名中级以上职称的MRI医师双盲法阅片并达成一致意见。结果 80个髌软骨均获得了满意的FA伪彩图,分为3层结构,呈蓝色-绿色-蓝色,关节下骨呈深蓝色,关节液呈红色,三层结构的FA平均值为:表层(0.436±0.091)ms,中间层(0.418±0.097)ms,深层(0.456±0.095)ms,三者比较显示深层FA最高,中间层FA最低,表层居中。结论研究健康人髌软骨DTI成像,测量FA值,对早期诊断髌软骨损伤和监测病变进展临床意义较大,为进一步研究髌软骨病变奠定了基础。
Objective To investigate the changes of FA value of patellar cartilage in magnetic resonance diffusion tensor imaging (DTI) in healthy adults. Methods Forty 80 knees of healthy volunteers were selected. The data were sent to GE workstation (ADW4.6) using GE Discovery MRI 750 3.0T magnetic resonance scanner DTI imaging. The FA pseudo-patella cartilage produced by Functool Color map, select the medial femoral condyle, lateral condyle, the femur patellar surface of the three regions of interest for the measurement of the cartilage surface area, the middle layer, the depth of FA values measured repeatedly for 3 times, take the average, from 3 MRI of mid-level and above titles of doctors double-blind reading and agreed. Results Eighty patellar cartilages were obtained satisfactory colorimetric maps of FA, which were divided into three layers with blue-green-blue color, dark blue in the articular surface and red in the synovial fluid. The FA of the three-layer structure was (0.436 ± 0.091) ms, the middle layer (0.418 ± 0.097) ms and the deep layer (0.456 ± 0.095) ms. The comparison among the three shows that FA is the highest in the deep layer, lowest in the middle layer, and middle in the middle layer. Conclusion DTI imaging of patellar cartilage in healthy people and measurement of FA value are of great significance for the early diagnosis of patellar cartilage injury and monitoring of disease progression, which lays the foundation for the further study of patellar cartilage lesions.
