目的观察静脉用药及局部注射新辅助化疗在宫颈癌治疗中的疗效及临床意义。方法对67例Ⅰb2~Ⅱb局部晚期宫颈癌先进行新辅助化疗(标准PVB方案,即卡铂、长春新碱、平阳霉素;静脉用药加5-FU宫颈局部注射)2~3个疗程,评价化疗效果,对可手术治疗的病例行广泛性子宫切除及盆腔淋巴结清扫术,不能手术者则行根治性放疗。结果化疗后65例病灶明显缩小,进行了手术治疗,术后病理检查结果阴道切缘及脉管均未见癌浸润,淋巴结转移2例。2例病情改善不满意,仍不能手术治疗,即行根治性放疗,无病情进展病例。结论静脉用药加局部注射进行新辅助化疗安全、有效,作为宫颈癌的一种新治疗手段,有显著的临床意义。
Objective To observe the curative effect and clinical significance of intravenous drug and neoadjuvant chemotherapy in the treatment of cervical cancer. Methods Neoadjuvant chemotherapy (standard PVB regimen: carboplatin, vincristine, pingyangmycin; intravenous injection plus topical injection of 5-FU cervix) was given to 67 patients with stage Ⅰb2 ~ Ⅱb locally advanced cervical cancer for evaluation. Chemotherapy effect, the cases of surgical treatment of extensive hysterectomy and pelvic lymph node dissection, who can not surgery radical radiotherapy. Results After chemotherapy, 65 lesions were significantly reduced, and the operation was performed. There was no carcinoma infiltration and lymph node metastasis in the vaginal margins and vessels in the pathological examination postoperatively. 2 patients were not satisfied with the improvement of the condition, still can not be treated, that is, radical radiotherapy, disease progression-free cases. Conclusion Intravenous drug plus local injection neoadjuvant chemotherapy is safe and effective, as a new treatment of cervical cancer, has significant clinical significance.
