椎管内恶性肿瘤22例临床分析

[目的]探讨诊断治疗椎管内恶性肿瘤的经验。[方法]回顾分析2000年1月至2010年1月间22例手术治疗的椎管内恶性肿瘤患者的临床资料。[结果]椎管内良、恶性肿瘤临床表现有一定差异。X线、CT、MRI确诊率分别为0、75.0%和100%。MRI诊断病理符合率为45.5%。22例患者完成手术,无严重手术并发症。术后感觉及运动功能恢复或基本恢复15例(68.2%)。复发8例(36.4%)。全组1、5和10年生存率分别为86.7%、51.9%和13.6%。[结论]CT与MRI对诊断有重要价值。怀疑椎管内肿瘤为恶性应早期积极进行以手术为主的综合治疗。 [Objective] To explore the diagnosis and treatment of malignant tumors in spinal canal. [Methods] The clinical data of 22 patients with spinal canal malignancy treated surgically from January 2000 to January 2010 were retrospectively analyzed. [Results] The clinical manifestations of benign and malignant tumors in spinal canal were different. X-ray, CT, MRI diagnosis rates were 0,75.0% and 100%. MRI diagnostic pathology coincidence rate was 45.5%. Twenty-two patients underwent surgery without serious surgical complications. 15 cases (68.2%) recovered or basically recovered postoperative feeling and motor function. Recurrent in 8 cases (36.4%). The 1, 5 and 10-year overall survival rates were 86.7%, 51.9% and 13.6%, respectively. [Conclusion] CT and MRI have important value in diagnosis. Suspected malignant tumor of the spinal canal should be actively carried out early surgery-based comprehensive treatment.
