目的探讨冲击波联合中药热敷对肩周炎患者的影响。方法将2013年1月—2016年1月的96例肩周炎患者随机分为中药热敷组(n=32)、冲击波组(n=31)、冲击波中药热敷组(n=33),观察两组治疗效果、视觉模拟标尺法(visual analogue scale,VAS)评分及运动功能评分。计量资料采用单因素方差分析,组间比较采用SNK法,计数资料采用χ2检验,P<0.05为差异有统计学意义。结果治疗后中药热敷组、冲击波组、冲击波中药热敷组总有效率(84.3%、77.4%、93.9%)比较差异均有统计学意义(均P<0.05)。治疗后冲击波中药热敷组VAS评分明显低于冲击波组和中药热敷组[(1.12±1.09)、(2.11±1.08)、(1.87±1.05)分],内旋、外旋、摸背、摸耳评分明显高于冲击波组和中药热敷组[(77.3±7.9)、(65.2±9.7)、(75.2±8.3)、(69.3±10.6)、(60.6±7.2)、(50.7±9.3)、(61.2±8.7)、(51.4±8.6)、(68.7±7.5)、(56.3±8.3)、(68.4±9.2)、(60.2±9.3)分],对比差异均有统计学意义(均P<0.05)。结论冲击波结合中药热敷能有效地缓解肩关节疼痛和关节功能,改善单一疗法有限的临床效果。
Objective To investigate the impact of shock wave combined with traditional Chinese medicine on patients with frozen shoulder. Methods One hundred and sixty-six patients with frozen shoulder from January 2013 to January 2016 were randomly divided into two groups: Chinese hot compress group (n = 32), shock wave group (n = 31) and shock wave Chinese hot compress group (n = 33) Group treatment effect, visual analogue scale (VAS) score and motor function score. The measurement data were analyzed by one-way ANOVA, SNK method was used to compare between groups, count data were analyzed byχ2 test, P <0.05 was considered statistically significant. Results After treatment, the total effective rate (84.3%, 77.4%, 93.9%) in the traditional Chinese medicine hot compress group, the shock wave group and the shock wave Chinese herbal compress group were significantly different (all P <0.05). After treatment, the VAS score of the Chinese medicine hot compress group was significantly lower than that of the shock wave group and the traditional Chinese medicine hot compress group [(1.12 ± 1.09), (2.11 ± 1.08), (1.87 ± 1.05)], internal rotation, external rotation, (77.3 ± 7.9), (65.2 ± 9.7), (75.2 ± 8.3), (69.3 ± 10.6), (60.6 ± 7.2), (50.7 ± 9.3), (61.2 ± 8.7) ), (51.4 ± 8.6), (68.7 ± 7.5), (56.3 ± 8.3), (68.4 ± 9.2) and (60.2 ± 9.3) points, respectively. The difference was statistically significant (all P <0.05). Conclusion Shockwave combined with traditional Chinese medicine hot compress can effectively relieve shoulder pain and joint function and improve the limited clinical effect of single therapy.
