目的探讨妊娠期甲状腺功能减退和妊娠期糖尿病(gestational diabetes mellitus,GDM)对孕妇血糖和血脂的影响。方法选择2013年1月-2014年12月富阳区妇幼保健院妇产科妊娠期甲状腺功能减退合并GDM孕妇68例作为A组,妊娠期甲状腺功能减退孕妇140例作为B组,GDM孕妇140例作为C组,健康孕妇140例作为对照组,测定各组孕妇血清甲状腺功能、血糖和血脂水平。结果 A组和B组的促甲状腺激素(Thyroid-stimulating hormone,TSH)水平明显高于C组和D组(P<0.05);A组和B组的游离甲状腺素(Free thyroxine,FT4)水平明显低于C组和D组(P<0.05);A组和C组空腹血糖、1 h血糖、2 h血糖均高于B组和D组(P<0.05),A组2 h血糖低于C组2 h血糖(P<0.05);A组高密度脂蛋白低于C组(P<0.05),A组甘油三酯高于B组、C组和D组(P<0.05);B组低密度脂蛋白高于C组(P<0.05),B组高密度脂蛋白低于D组(P<0.05),B组甘油三酯高于D组(P<0.05);C组低密度脂蛋白低于D组(P<0.05),C组总胆固醇低于D组(P<0.05)。结论妊娠期甲状腺功能减退和GDM孕妇存在血糖和血脂异常,妊娠期甲状腺功能减退和GDM相互影响,加重血糖和血脂水平异常。
Objective To investigate the effects of gestational diabetes mellitus (gestational diabetes mellitus, gestational diabetes mellitus, gestational diabetes mellitus, gestational diabetes mellitus on blood glucose and blood lipid in pregnant women. Methods Select January 2013-December 2014 Fuyang District MCH obstetrics and Gynecology during pregnancy hypothyroidism 68 cases of GDM pregnant women as a group of 140 pregnant women with hypothyroidism during pregnancy as Group B 140 pregnant women as GDM Group C, healthy pregnant women 140 cases as the control group, determination of serum thyroid function, blood glucose and blood lipid levels in each group. Results The levels of Thyroid-stimulating hormone (TSH) in group A and group B were significantly higher than those in group C and D (P <0.05). The level of free thyroxine (FT4) in group A and group B was significantly (P <0.05). The fasting blood glucose, 1 hour blood glucose and 2 hour blood glucose in group A and group C were higher than those in group B and group D (P <0.05), and the blood glucose in group A was lower than that in group C (P <0.05); Group A high density lipoprotein was lower than Group C (P <0.05), Group A triglyceride was higher than Group B, Group C and Group D (P <0.05); Group B (P <0.05), high density lipoprotein of group B was lower than that of group D (P <0.05), triglyceride of group B was higher than that of group D (P <0.05), and low density lipoprotein of group C (P <0.05), total cholesterol in group C was lower than that in group D (P <0.05). Conclusion Hypothyroidism during pregnancy and GDM pregnant women with abnormal blood glucose and dyslipidemia, hypothyroidism during pregnancy and GDM mutual influence, aggravating abnormal blood glucose and blood lipid levels.
