目的对比分析右美托咪定与咪达唑仑复合舒芬太尼用于肝癌微波治疗术患者的麻醉效果。方法 160例行肝癌微波治疗术患者,根据数字随机表法分为咪达唑仑组和右美托咪定组,各80例。咪达唑仑组采用咪达唑仑复合舒芬太尼麻醉,右美托咪定组采用右美托咪定复合舒芬太尼麻醉。比较两组患者麻醉效果、舒芬太尼自控镇痛(PCA)按压次数、平均有效按压次数以及患者心动过速、高血压、呼吸抑制等并发症发生情况。结果右美托咪定组麻醉总有效率为92.50%,高于咪达唑仑组的75.00%,差异具有统计学意义(P<0.05)。两组患者心动过速、高血压、低血压和心动过缓发生情况比较差异无统计学意义(P>0.05);右美托咪定组呼吸抑制发生情况少于咪达唑仑组,差异具有统计学意义(P<0.05)。右美托咪定组舒芬太尼PCA按压次数、平均有效按压次数均少于咪达唑仑组,差异具有统计学意义(P<0.05)。结论右美托咪定复合舒芬太尼用于肝癌微波治疗术患者麻醉效果优于咪达唑仑复合舒芬太尼,可有效提高麻醉效果,减少舒芬太尼PCA按压次数、平均有效按压次数,减少呼吸抑制的发生,安全有效,值得推广。
Objective To compare and analyze the anesthetic effects of dexmedetomidine combined with midazolam and sufentanil for microwave therapy of liver cancer. Methods A total of 160 patients undergoing microwave therapy of liver cancer were divided into midazolam group and dexmedetomidine group according to the digital random table method, 80 cases each. Midazolam group was treated with midazolam combined with sufentanil, dexmedetomidine group with dexmedetomidine combined with sufentanil anesthesia. The anesthesia effects, the number of times of sufentanil-controlled analgesia (PCA), the number of effective compressions, and the incidence of complications such as tachycardia, hypertension and respiratory depression were compared between the two groups. Results The total effective rate of dexmedetomidine group was 92.50%, higher than 75.00% of midazolam group, the difference was statistically significant (P <0.05). There was no significant difference in the incidence of tachycardia, hypertension, hypotension and bradycardia between the two groups (P> 0.05). The incidence of respiratory depression was lower in the dexmedetomidine group than in the midazolam group Statistical significance (P <0.05). The number of PCA pressures and the average number of effective pressures of sufentanil in dexmedetomidine group were less than those in midazolam group, with significant difference (P <0.05). Conclusion Dexmedetomidine combined with sufentanil is superior to midazolam combined with sufentanil in microwave therapy of liver cancer, which can effectively improve the anesthetic effect, reduce the number of PCA pressing sufentanil, the average effective compression Frequency, reduce the occurrence of respiratory depression, safe and effective, it is worth promoting.
