目的:探讨肾透明细胞癌(CRCC)的螺旋CT(SCT)多期增强表现与癌组织中环氧化酶-2(COX-2)表达及其微血管密度(MVD)之间的关系。方法:收集经手术病理确诊且术前有详细SCT资料的CRCC石醋包埋切片32例和正常肾组织切片10例,采用免疫组化SP法检测Ⅷ因子相关抗原和COX-2的表达。观察SCT图像上平扫、CMP、NP和EP时肿瘤密度及肿瘤最大强化程度等多期增强指标。结果:CRCC组织中,MVD明显高于正常肾组织,U=7.148,P=0.000;COX-2表达也高于正常肾组织,χ2=9.773,P=0.002。COX-2表达程度与MVD之间有显著相关性,rs=0.739,P=0.003。CRCC癌组织的最大强化程度、强化各期CT值与COX-2表达程度和MVD呈正相关。结论:CRCC的SCT多期增强表现可以无创伤评价肿瘤血管生成,在一定程度上反映COX-2的表达。
Objective: To investigate the relationship between the multiphasic enhancement of spiral CT (SCT) in renal clear cell carcinoma (CRCC) and the expression of cyclooxygenase-2 (COX-2) and its microvessel density (MVD) Methods: Thirty-two cases of CRCC and 15 cases of normal renal tissue confirmed by surgery and pathology were collected. The expressions of factor Ⅷ related antigen and COX-2 were detected by immunohistochemical SP method. Observation of SCT images of plain scan, CMP, NP and EP tumor density and maximum degree of tumor enhancement and other multi-phase enhancement indicators. Results: The MVD in CRCC was significantly higher than that in normal renal tissue (U = 7.148, P = 0.000). The expression of COX-2 in CRCC was also higher than that in normal renal tissue (χ2 = 9.773, P = 0.002). There was a significant correlation between the expression of COX-2 and MVD, rs = 0.739, P = 0.003. The maximum degree of enhancement of CRCC and the enhancement of CT value in each stage were positively correlated with the expression of COX-2 and MVD. CONCLUSIONS: The multi-phase SCT of CRCC can assess tumor angiogenesis at non-invasive stage, which reflects the expression of COX-2 to a certain extent.
