目的 探讨T淋巴细胞亚群以及Th1/Th2相关细胞因子在特发性血小板减少性紫癜 (ITP)中的变化。方法 用PMA、ionomycin作刺激剂 ,刺激全血中淋巴细胞表达细胞因子 ,以monensin阻断细胞因子分泌至胞外 ,应用两种免疫荧光抗体同时标记淋巴细胞的膜表面特异的分子和表达被阻滞在细胞内的细胞因子 ,流式细胞仪进行检测和分析。结果 ITP患儿外周血CD4 + 降低 ,CD8+ 升高 ,CD4 + /CD8+降低。ITP患儿IFN γ血清表达升高 ,Th1细胞占 2 0 6 6 %± 2 82 % ,IL 4表达降低 ,Th2细胞占 4 5 5 %± 2 33% ,Th1/Th2的比值为 4 78± 0 6 6 ,明显高于健康儿童的 1 4 3± 0 19(P <0 0 5 )。结论 ITP患儿T淋巴细胞亚群表达的比例失调 ,IFN γ产生增多 ,IL 4减少 ,提示辅助性T淋巴细胞亚型 (Th1/Th2 )失调
Objective To investigate the changes of T lymphocyte subsets and Th1 / Th2 related cytokines in idiopathic thrombocytopenic purpura (ITP). Methods PMA and ionomycin were used as stimulants to stimulate the expression of cytokines in lymphocytes in whole blood. Monensin was used to block the secretion of cytokines to the extracellular matrix. Two kinds of immunofluorescent antibodies were used to simultaneously label the membrane-surface-specific molecules of lymphocytes and their expression Hysteresis in the cells of cytokines, flow cytometry for detection and analysis. Results The levels of CD4 +, CD8 + and CD4 + / CD8 + in peripheral blood of ITP children decreased. The expression of IFN-γ in children with ITP was increased, with Th1 cells accounting for 26.6% ± 82.2%, IL 4 expression being reduced, Th2 cells accounting for 45.5 ± 23.3%, and Th1 / Th2 ratios being 4 78 ± 0 6 6, which was significantly higher than that of healthy children (143 ± 0 19, P <0 05). Conclusions The proportion of T lymphocyte subsets in ITP children is imbalanced, the production of IFNγ is increased, and IL 4 is decreased, which indicates the imbalance of Th1 / Th2 helper T lymphocyte subsets
