目的探讨主动脉内球囊反搏术(IABP)在高危冠心病患者介入治疗中的有效性。方法对我院2016年5月至2017年2月70例高危冠心病患者的临床资料进行回顾性分析,将入组的患者依据随机抽签原则分为对照组和研究组,每组35例,对照组患者未置入IABP或病情恶化后被动置入IABP,研究组患者在PCI术前常规预防性置入IABP,比较两组患者术中及术后情况。结果研究组患者术后平均动脉压、术后TIMIⅢ级血流比例显著高于对照组(P<0.05),贫血、坠积性肺炎以及下肢栓塞的发生率高于对照组(P<0.05),住院时间长于对照组(P<0.05),心力衰竭、心源性休克以及恶性心律失常的发生率显著少于对照组(P<0.05),1年内研究组不良血管事件发生率显著低于对照组,两组比较差异有统计学意义(P<0.05)。结论预防性的应用IABP能有效改善高危冠心病患者血流动力学指标,减少介入治疗的并发症,提高患者的生存质量,具有重要的临床应用价值。
Objective To investigate the effectiveness of intra-aortic balloon pump (IABP) in the interventional treatment of high-risk patients with coronary heart disease. Methods The clinical data of 70 high-risk CHD patients from May 2016 to February 2017 in our hospital were retrospectively analyzed. According to the principle of random lottery, the patients were divided into control group and study group, with 35 cases in each group. Group IABP patients were not placed or worsen after the passive placement of IABP, study group patients preoperative PCI preoperative IABP, compared two groups of patients during and after surgery. Results The mean postoperative arterial pressure and postoperative TIMI grade Ⅲ blood flow ratio in the study group were significantly higher than those in the control group (P <0.05). The incidence of anemia, hypostatic pneumonia and lower extremity embolism were higher in the study group than in the control group (P <0.05) The hospitalization time was longer than that of the control group (P <0.05). The incidence of heart failure, cardiogenic shock and malignant arrhythmia was significantly less than that of the control group (P <0.05). The incidence of adverse vascular events in the study group was significantly lower than that in the control group , The difference between the two groups was statistically significant (P <0.05). Conclusions The prophylactic application of IABP can effectively improve the hemodynamic parameters of patients with high-risk coronary heart disease, reduce the complications of interventional therapy and improve the quality of life of patients with important clinical value.
