目的应用实时三维超声心动图(RT-3DE)和斑点追踪技术(STI)评价Ⅱ型糖尿病(2DM)患者无心血管疾病症状期左房(LA)结构和功能的变化。方法收集37例2DM患者和30例健康对照者,应用RT-3DE获取LA最大容积(Vmax)、最小容积(Vmin)及收缩前容积(Vpre A),并计算LA总射血分数(TEF)、膨胀指数(EI)、主动射血分数(AEF)及被动射血分数(PEF)。应用STI获取LA负向峰值应变(NS)、正向峰值应变(PS)。比较两组统计学差异。结果 2DM组LAVmax、LAVmin及Vpre A显著增加,LATEF、LAEI及LAPEF显著减低;LAAEF显著增加,LANS绝对值显著增加,LAPS值显著减低。结论 2DM患者在无心血管疾病症状期LA的结构和功能已受到影响,进而影响LA对左室舒张功能的代偿调节作用。
Objective To evaluate the changes of structure and function of left atrium (LA) in patients with type 2 diabetes mellitus (T2DM) without cardiovascular disease by real-time three-dimensional echocardiography (RT-3DE) and speckle tracking (STI) Methods Totally 37 patients with 2DM and 30 healthy controls were recruited. The maximum and minimum Vmax and Vpre A were obtained by RT-3DE. The LA total ejection fraction (TEF) EI, AEF and PEF. LA negative peak strain (NS) and positive peak strain (PS) were obtained using STI. Compare the two groups of statistical differences. Results LAVmax, LAVmin and Vpre A increased significantly in 2DM group, LATEF, LAEI and LAPEF decreased significantly; LAAEF increased significantly, absolute value of LANS increased significantly, LAPS value decreased significantly. Conclusions The structure and function of LA in patients with 2DM without symptoms of cardiovascular disease have been affected, which may affect the compensatory regulation of LA on left ventricular diastolic function.
