1998年3月4日,位于美国路易斯安那州的Sonat勘探开发公司的油气生产设施发生一起灾难性的容器失效和火灾事故。事故造成容器附近的4名工人死亡,生产设施遭到严重损坏。本文结合美国化学品安全与危害调查委员会(CSB)对该起事故的调查,从技术和管理角度综合分析、总结事故发生的直接原因和间接原因,并给出CSB调查组的建议。
On March 4, 1998, a catastrophic container failure and fire accident occurred at the oil and gas production facilities of Sonat Exploration and Development Company in Louisiana, USA. Four workers near the container died in the accident and the production facilities were severely damaged. Based on a survey conducted by the U.S. Commission for Chemical Safety and Hazard Investigation (CSB) on this accident, this article analyzes the causes of accidents and their causes indirectly from a technical and management perspective and gives suggestions of the CSB investigation team.
