目的探究高频超声在类风湿性关节炎(RA)腕关节早期病变中的诊断价值。方法 64例RA患者作为实验组,64例健康人作为对照组,分别对两组进行腕关节高频超声检查,实验组同时进行类风湿因子(RF)检测,观察两组高频超声结果 ,并比较实验组采用高频超声与RF检测的阳性率。结果 64例RA患者均有滑膜增生,56例关节积液,28例关节周围炎症渗出,30例血管翳,23例骨质侵蚀;有47例RA患者呈现不均匀低回声,17例不均匀高回声。对照组平均滑膜厚度为(1.64±0.25)mm,明显低于实验组的(3.52±1.12)mm,差异具有统计学意义(t=13.106,P<0.05)。经相关性分析表明,滑膜动脉阻力指数(RI)与滑膜血流分级呈负相关(r=-0.69,P<0.05);搏动指数(PI)与滑膜血流分级无明显相关性(P>0.05)。高频超声检查的阳性率为100.00%,明显高于RF检测的43.75%,差异具有统计学意义(χ~2=50.087,P<0.05)。结论高频超声在类风湿关节炎患者中腕关节早期病变具有较高的检出率,更为准确的反映患者的病理改变,为临床治疗提供参考意见。
Objective To investigate the diagnostic value of high frequency ultrasound in early stage of rheumatoid arthritis (RA) wrist joint. Methods Sixty-four patients with RA were selected as the experimental group and 64 healthy individuals as the control group. High-frequency ultrasonography was performed on the two groups. The experimental group was also given rheumatoid factor (RF) test. The results of high-frequency ultrasound The experimental group compared the positive rate of high-frequency ultrasound and RF detection. Results There were synovial hyperplasia, synovial hyperplasia in 56 cases, joint effusion in 28 cases, exudation of inflammation around the joint in 28 cases, angiogenesis in 30 cases and bone erosion in 23 cases. There were 47 patients with RA presenting with uneven hypoechoicity and 17 cases without Even high echo. The average synovial thickness of the control group was (1.64 ± 0.25) mm, which was significantly lower than that of the experimental group (3.52 ± 1.12) mm, the difference was statistically significant (t = 13.106, P <0.05). Correlation analysis showed that the synovial artery resistance index (RI) was negatively correlated with synovial blood flow classification (r = -0.69, P <0.05). There was no significant correlation between pulsatility index (PI) P> 0.05). The positive rate of high frequency ultrasound examination was 100.00%, which was significantly higher than that of RF detection (43.75%). The difference was statistically significant (χ ~ 2 = 50.087, P <0.05). Conclusion High-frequency ultrasound in patients with rheumatoid arthritis early wrist lesions with a higher detection rate, more accurately reflect the pathological changes in patients with clinical treatment for reference.
