目的了解沈阳市某高校职工不同诊断标准代谢综合征的患病状况,为修订标准和防治代谢综合征提供参考。方法以沈阳市某高校1 907名在职及退休职工为研究对象,进行人体测量、生理及血液指标检测。结果根据中华糖尿病学会标准(CDS标准)、美国国家胆固醇教育计划成人治疗指南Ⅲ修订标准(ATPⅢ修订标准)及国际糖尿病联盟标准(IDF标准),代谢综合征的粗患病率分别为22.2%3,5.5%及32.3%,标准化患病率为16.6%2,6.8%及23.8%,CDS标准下粗患病率、标准化患病率低于其他标准(P值均<0.01)。应用各标准,男性患病率高于女性,男、女患病率随年龄的增长而升高;在单项指标中,肥胖率较高;高三酰甘油、肥胖使代谢综合征的危险度明显升高。CDS标准与ATPⅢ修订标准及IDF标准的一致性中等,Kappa系数分别为0.64,0.62;调整年龄和性别后,BMI正常而腰围异常者患代谢综合征的危险度是2项均正常者的2.48倍(OR=2.489,5%CI为1.59~3.86,P<0.01)。结论该高校职工代谢综合征的患病率处于较高水平;3种标准对代谢综合征的诊断有差异,CDS标准诊断的患病率最低。
Objective To understand the prevalence of metabolic syndrome in different diagnostic criteria of workers in a university in Shenyang City, and to provide reference for revision of standards and prevention and treatment of metabolic syndrome. Methods A total of 1 907 on-the-job and retired workers in a certain university in Shenyang were enrolled in this study, and their body measurements, physiology and blood indexes were tested. Results The crude prevalence rates of metabolic syndrome were 22.2%, 3 according to the Chinese Diabetes Association Standard (CDS), the American National Cholesterol Education Program Adult Treatment Guidelines Ⅲ Revision Standard (ATP Ⅲ revised standard) and the International Diabetes Federation standard (IDF standard) , 5.5% and 32.3% respectively. The standardized prevalence rates were 16.6%, 2.6% and 23.8%, respectively. The crude prevalence under the CDS standard was lower than the other criteria (P <0.01). The prevalence rates of males and females were higher than those of females. The prevalence of males and females increased with advancing age. The rates of obesity were higher in the single indicators. The risk of metabolic syndrome was significantly increased with hyper-triglycerides and obesity high. CDS standard and ATP Ⅲ revised standards and IDF standard consistency, Kappa coefficient was 0.64,0.62; adjusted for age and gender, normal BMI and waist circumference were risk of metabolic syndrome were 2. 2 times normal (OR = 2.489, 5% CI 1.59 ~ 3.86, P <0.01). Conclusions The prevalence of metabolic syndrome in the college staff is at a high level. There are differences in the diagnosis of metabolic syndrome between the three standards, and the lowest prevalence of CDS standard diagnosis.
