目的为了探讨自身免疫性甲状腺疾病(AITD)血清瘦素(Lep)水平变化与甲状腺激素及能量代谢之间关系。方法对158例AITD患者用放射免疫分析法(RIA)测定Lep、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、甲状腺球蛋白抗体(TGA)和甲状腺微粒体抗体(TMA);用免疫放射分析法(IRMA)测促甲状腺激素(TSH);用放射受体分析法(RRA)测促甲状腺激素受体抗体(TRAb)及常规测量体重指数(BMI),并取38例甲状腺功能正常体检者作对照。结果AITD甲减组Lep明显低于对照组(P<0.01)、AITD甲亢组BMI低于AITD甲减组(P<0.05)。在对照组、AITD甲亢组、AITD甲减组、AITD甲功正常组中Lep水平皆与BMI呈显著正相关(r分别为0.52、0.48、0.50、0.55)AITD甲减组Lep于FT4相关(r=0.31,P<0.05);与TSH负相关(r=-0.39,P<0.05)。结论甲状腺激素的不同变化可影响Lep的正常调节及能量代谢。
Objective To investigate the relationship between serum leptin (Lep) level and thyroid hormone and energy metabolism in autoimmune thyroid disease (AITD). Methods Fifty-eight patients with AITD were analyzed by radioimmunoassay (RIA) for Lep, free triiodothyronine (FT3), free thyroxine (FT4), thyroglobulin antibody (TGA) and thyroid microsomal antibody (TMA) Thyroid hormone (TSH) was measured by radioimmunoassay (IRMA); Thyroid hormone receptor antibody (TRAb) and routine body mass index (BMI) were measured by radioimmunoassay (RRA) Function of normal physical examination as a control. Results The Lep in AITD hypothyroidism group was significantly lower than that in control group (P <0.01). The BMI in AITD hyperthyroidism group was lower than that in AITD hypothyroidism group (P <0.05). In the control group, AITD hyperthyroidism group, AITD hypothyroidism group, AITD normal thyroid function group Lep levels were positively correlated with BMI (r were 0.52,0.48,0.50,0.55) AITD hypothyroidism Lep in FT4-related (r = 0.31, P <0.05), negatively correlated with TSH (r = -0.39, P <0.05). Conclusion Different changes of thyroid hormone can affect the normal regulation of Lep and energy metabolism.
