目的:探讨化痰散结、活血解毒法对非酒精性脂肪肝模型大鼠血清学指标(肝功能、血脂、HOMA-IR、硫化氢)及肝脏组织病理学的影响,验证其疗效确切性,并探讨其可能的作用机制。方法:SPF级雄性Wistar大鼠50只随机分为空白对照组12只和模型组38只,空白对照组予普通饲料喂养,模型组予高脂饲料喂养,喂养8周后于模型组随机剖杀4只取肝组织行病理学观察达到中-重度脂肪肝的诊断标准,随机将模型组分为中药高剂量组11只、中药低剂量组11只和模型对照组(予生理盐水灌胃)12只,灌胃8周后观察大鼠肝功能、血脂、HOMA-IR、硫化氢和肝脏病理学指标变化。结果:模型对照组大鼠ALT、AST、TC、TG、HOMA-IR均较空白对照组升高且差异有统计学意义(**P<0.01),大鼠血清H2S浓度较空白对照组降低且差异有统计学意义(**P<0.01),中药高、低剂量组在降低血清ALT、AST、TG、TC和HOMA-IR以及升高血清H2S浓度均较模型对照组差异有统计学意义,且在改善血TC、TG和HOMA-IR方面高剂量组优于低剂量组(○P<0.05)。结论:化痰散结、活血解毒法可改善NAFLD大鼠肝脏脂肪沉积和肝组织病理学表现,疗效确切,本法可纠正糖脂代谢紊乱,改善胰岛素抵抗,升高NAFLD大鼠血清H2S浓度,改善肝脏微循环,抑制肝星状细胞活化和增殖,延缓肝纤维化进程,利于疾病恢复。
Objective: To investigate the effect of Huatan Sanjie and Huoxue Jiedu (Huatan Jiedu) on the serum indexes (liver function, blood lipid, HOMA-IR, H2S) and liver histopathology in nonalcoholic fatty liver disease model rats to verify its curative effect, And explore its possible mechanism of action. Methods: Fifty SPF male Wistar rats were randomly divided into blank control group (n = 12) and model group (n = 38). The blank control group was fed with normal diet. The model group was fed with high fat diet. After 8 weeks of feeding, Four liver tissues were obtained and pathologically observed to reach the diagnostic criteria of moderate-severe fatty liver. The model group was randomly divided into 11 groups of high-dose Chinese medicine group, 11 low-dose Chinese medicine group and model control group (intragastrically with saline) 12 Only after 8 weeks of administration, liver function, blood lipid, HOMA-IR, hydrogen sulfide and hepatic pathological changes were observed. Results: The levels of ALT, AST, TC, TG and HOMA-IR in the model control group were significantly higher than those in the blank control group (** P <0.01), and the concentrations of serum H2S in the model control group were lower than those in the blank control group The difference was statistically significant (** P <0.01). The Chinese medicine high and low dose groups had statistically significant differences compared with the model control group in decreasing serum ALT, AST, TG, TC and HOMA-IR and increasing serum H2S concentration, The high-dose group was superior to the low-dose group in improving blood TC, TG and HOMA-IR (P P <0.05). Conclusion: Huatan Sanjie, Huoxue Jiedu method can improve NAFLD rat liver fat deposition and liver histopathology, the exact effect, this method can correct the disorders of glucose and lipid metabolism, improve insulin resistance, increase serum H2S concentration in NAFLD rats, Improve liver microcirculation, inhibit hepatic stellate cell activation and proliferation, delay the process of liver fibrosis, which will benefit the recovery of the disease.
