目的观察直肠癌病例手术后标本缺氧诱导因子1α(HIF-1α)表达水平与血管内皮生长因子(VEGF)、临床病理特征的相关性以及在判断直肠癌预后中的价值。方法应用免疫组化技术检测30例直肠癌手术后标本HIF-1α和VEGF的表达,同时了解术后病理特征和随访治疗结果。结果HIF-1α表达弱阳性和强阳性者分别为11例和19例,VEGF表达弱阳性和强阳性者分别为16例和14例,两者间存在显著的相关性(P<0.01);HIF-1α的表达水平与Dukes分期和淋巴结转移情况之间也有显著的相关性(P<0.01和P<0.01),而与肿瘤病理分化程度之间则无明显的统计学意义(P>0.05)。3年生存率为51.0%,其中HIF-1α表达为弱阳性和强阳性者分别为90.9%和34.4%,两者间有显著差异(P<0.01)。COX多因素分析显示HIF-1α的表达水平是影响直肠癌生存率的重要预后因素(P<0.05)。结论HIF-1α的表达水平是影响直肠癌预后的重要因素,临床上值得进一步研究。
Objective To observe the correlation between the expression of hypoxia inducible factor 1α (HIF-1α) and vascular endothelial growth factor (VEGF), clinicopathological features and the prognosis of rectal cancer after operation in patients with rectal cancer. Methods Immunohistochemical technique was used to detect the expression of HIF-1α and VEGF in 30 specimens of rectal cancer after surgery. The pathological features and follow-up results were also studied. Results HIF-1α expression was weakly positive and strongly positive in 11 and 19 cases, respectively. There were 16 cases and 14 cases with weakly positive and strongly positive VEGF expression, respectively. There was a significant correlation between them (P <0.01) (P <0.01 and P <0.01), but not with the degree of tumor pathological differentiation (P> 0.05) .Conclusion: There is a significant correlation between the expression level of Dukes and Dukes stage and lymph node metastasis. The 3-year survival rate was 51.0%. Among them, the expression of HIF-1α was weakly positive and strongly positive were 90.9% and 34.4% respectively, with significant difference (P <0.01). Cox multivariate analysis showed that the expression of HIF-1α was an important prognostic factor for the survival of rectal cancer (P <0.05). Conclusion The expression of HIF-1α is an important factor influencing the prognosis of rectal cancer. It is worth further study in clinic.
