目的:探讨口腔种植修复对牙列缺失患者咀嚼功能及龈沟液单核细胞趋化蛋白-1(MCP-1)和肿瘤坏死因子-α(TNF-α)表达的影响。方法:选择浙江省桐乡市第一人民医院2017年6月至2019年6月收治的牙列缺失患者98例,其中49例患者行口腔种植修复治疗(观察组),49例患者行常规修复治疗(对照组)。两组均随访6个月评价疗效,比较治疗前和治疗后6个月咀嚼功能、语言功能、固位功能、生命质量及龈沟液MCP-1和TNF-α水平变化。结果:观察组总有效率高于对照组[95.92%(47/49)比79.59%(39/49)],差异有统计学意义(n χ2=6.078,n P<0.05)。观察组治疗后6个月咀嚼功能、语言功能和固位功能评分高于对照组(n P<0.05)。观察组治疗后6个月情感功能、社会功能和生理功能评分高于对照组(n P<0.05)。观察组治疗后6个月龈沟液MCP-1和TNF-α水平低于对照组[(32.09 ± 7.65)μg/L比(43.62 ± 9.23)μg/L、(2.19 ± 0.38)μg/L比(3.20 ± 0.51)μg/L](n P<0.05)。n 结论:口腔种植修复对牙列缺失患者疗效良好,且可改善患者咀嚼功能和生命质量,及降低龈沟液MCP-1和TNF-α表达。“,”Objective:To investigate the masticatory function and the expression of monocyte chemoattractant protein-1 (MCP-1) and tumor necrosis factor-α(TNF-α) in gingival crevicular fluid.Methods:The clinical data of 98 patients with dentition loss admitted to Tongxiang First People′s Hospital from June 2017 to June 2019 were analyzed, and 49 cases were treated with oral implant repair(observation group) and 49 cases were treated with conventional repair(control group). Both groups were followed up for 6 months. The masticatory function, speech function, retention function, quality of life and the changes of MCP-1 and TNF-α in gingival crevicular fluid were compared before treatment and 6 months after treatment.Results:The total effective rate of the observation group was higher than that of the control group: 95.92%(47/49) vs. 79.59%(39/49), and the difference was statistically significant (n χ2=6.078, n P<0.05). At 6 months after treatment, the scores of masticatory function, speech function and retention function in the observation group were higher than those in the control group (n P<0.05). The scores of emotional function, social function and physiological function in the observation group were higher than those in the control group (n P<0.05). The levels of MCP-1 and TNF-α in gingival crevicular fluid of the observation group were lower than those of the control group: (32.09 ± 7.65) μg/L vs. (43.62 ± 9.23) μg/L, (2.19 ± 0.38) μg/L vs. (3.20 ± 0.51) μg/L, and the differences were statistically significant (n P<0.05).n Conclusions:Dental implant repair is effective for patients with dentition loss, and it can improve masticatory function and quality of life, and reduce the expression of MCP-1 and TNF-α in gingival crevicular fluid.
