患儿,女,9个月。右眼红肿,发痒、流泪,分泌物增多半月,发现眼内白色线状小虫游动五天就诊。查体:右眼睑结膜充血明显,轻微肿胀,睑缘分泌物增多,内眦部轻度糜烂红肿。可见白色线伏虫子自结膜囊爬出活动,眼球转动或用电筒光照时,虫则钻入结膜囊内,翻开患眼眼睑,用手轻压固定眼球,使眼科小镊夹出小虫五条.虫体呈乳白色、线状,两端软细;较长者三条长约1.0cm,较短两条长约0.8cm,粗均约1mm,较短者尾略卷曲,虫子取出后,尚可摆动一分钟左右.标本经鉴定为“人眼结合膜吸吮线虫.”。讨论:此虫是狗猫等动物眼部常见寄生虫病。人体感染常见于夏秋季,多累及婴幼儿,可能为感染此虫的蝇舐吸终末宿主眼部而感染,常单眼发生。预防注意防止蝇类舐吮眼部及面部。
