目的研究创伤性急性肾功能衰竭(TARF)早期尿中脱落的肾小管上皮活细胞作为预警诊断指标的应用价值。方法休克加高渗甘油注射法所致新西兰大白兔TARF发生过程中,用台盼蓝拒染法对尿中肾小管上皮活细胞进行检测,分析其与肾功能变化的关系。结果TARF发生过程中,尿中排出肾小管上皮活细胞数量增加达0.4×106/h,其出现较血清尿素氮(BUN)升高早3小时,较血清肌酐(Cr)升高早2小时。结论尿中肾小管上皮活细胞在TARF发生过程的早期明显升高,可作为TARF早期预警诊断指标。
Objective To investigate the value of early aspiration in the diagnosis of tubular epithelial cells shedding from urine in the early stage of traumatic acute renal failure (TARF). Methods TARF in New Zealand white rabbits induced by shock plus hypertonic glycerol injection was used to detect the tubular epithelial viable cells in urine by trypan blue exclusion method. The relationship between them and the changes of renal function was analyzed. Results During TARF, the number of viable urothelial cells excreted in renal tubules increased by 0.4 × 106 / h, which was 3 hours earlier than that of serum BUN and 2 hours earlier than serum creatinine (Cr) hour. Conclusions Urine renal tubular epithelial live cells significantly increase in the early stage of TARF, which can be used as an early warning indicator of TARF.
