作者报告1986年3月至1995年12月对337例食管癌的治疗。其中手术175例(51.9%),切除癌肿165例,手术切除率94.3%。手术死亡率1.8%(3/165)。术后发生胸部吻合口瘘3例,颈部吻合口瘘4例,吻合口瘘发生率4.2%(7/165)。本组随访率为91.2%。1年、3年、5年生存率分别为73.6%、42.5%、31.9%。本组中晚期食管癌较多,采取以手术为主辅以术前放疗和术后化疗以及免疫治疗等措施,减少了复发或转移,确实能延长生存期,从而提高了生存率。作者认为,对于中上段食管癌应行全胸段食管切除颈部食管胃吻合术。
The authors report treatment of 337 cases of esophageal cancer from March 1986 to December 1995. Among them, 175 cases (51.9%) were surgically treated and 165 cases were resected. The surgical resection rate was 94.3%. The operative mortality rate was 1.8% (3/165). There were 3 cases of chest anastomotic fistula, 4 cases of neck anastomotic fistula, and anastomotic leakage rate of 4.2% (7/165). The follow-up rate of this group was 91.2%. The 1-year, 3-year, and 5-year survival rates were 73.6%, 42.5%, and 31.9%, respectively. In this group of patients with advanced esophageal cancer, the use of surgery as the main supplement and preoperative radiotherapy and postoperative chemotherapy and immunotherapy and other measures to reduce the recurrence or metastasis, can actually increase the survival period, thereby improving the survival rate. The authors believe that for the middle and upper esophageal cancer should be performed a full thoracic esophagectomy cervical esophagogastric anastomosis.
