目的探讨介入性超声对甲状腺占位性病变良恶性诊断的价值。方法通过对212例常规超声提示甲状腺内有结节样病灶,但不能明确病灶良恶性的患者,在超声引导下对可疑病灶进行穿刺、活检,并将病理检查结果与超声检查资料进行对比析。结果①本组212例患者穿刺活检全部成功,成功率为100%,无一例出现明显并发症。②病理检查显示:212例患者中占位性病变57例,占26.89%;其中甲状腺癌15例,占7.08%,甲状腺腺瘤42例,占19.81%。③病理结果与超声检查对比:有7例患者病理提示为甲状腺腺瘤或甲状腺癌,常规超声检查未能提示,漏诊率3.3%;30例患者病理结果为结节性甲状腺肿,超声诊断为甲状腺占位性病变,误诊率14.15%。结论超声引导下自动活检技术,可以对病灶进行选择性穿刺引导,操作方便,安全可靠,并发症低,是早期诊断甲状腺占位性病变有效地方法,具有临床广泛应用价值。
Objective To investigate the value of interventional ultrasonography in benign and malignant thyroidism lesions. Methods 212 patients with nodular lesions in the thyroid gland suggested by conventional ultrasound, but not clear benign and malignant lesions, ultrasound-guided suspicious lesions puncture and biopsy, and pathological examination results and ultrasound data were compared. Results ①There was a successful biopsy in 212 patients in this group with a success rate of 100%. No obvious complication occurred. ② Pathological examination showed 57 lesions (26.89%) in 212 patients, including 15 thyroid carcinoma (7.08%) and thyroid adenoma (42.8%), accounting for 19.81%. ③ pathological results and ultrasound examination: 7 cases of pathological tips for thyroid adenoma or thyroid cancer, conventional ultrasound failed to prompt, missed diagnosis rate of 3.3%; 30 cases of pathological results for nodular goiter, ultrasound diagnosis of thyroid Occupying lesions, misdiagnosis rate of 14.15%. Conclusion Ultrasound guided automatic biopsy technique can guide the selective puncture of the lesion. It is easy to operate, safe and reliable with low complication. It is an effective method to diagnose thyroid gland lesions in the early stage and has clinical value widely.
