背景与目的:临床Ⅰ期非小细胞肺癌是否应常规行纵隔镜检查仍存在争议,本研究旨在评估应用纵隔镜检查临床Ⅰ期非小细胞肺癌纵隔淋巴结转移情况的价值。方法:2000年10月至2004年2月,31例临床诊断为Ⅰ期非小细胞肺癌的患者接受经颈纵隔镜检查。结果:纵隔镜检查共发现2例N2期患者,1例N3期患者(右斜角肌前淋巴结转移),阳性率为10%(3/31);敏感性75%,特异性100%,准确率97%。结论:对临床Ⅰ期非小细胞肺癌进行纵隔镜检查有较大的意义,术前可常规行纵隔镜检查。
BACKGROUND & OBJECTIVE: It is still controversial whether clinical phase Ⅰ mediastinoscopy should be performed routinely in mediastinal non-small cell lung cancer. This study was designed to evaluate the value of mediastinoscopy in detecting mediastinal lymph node metastases in stage Ⅰ non-small cell lung cancer. METHODS: From October 2000 to February 2004, 31 patients with clinically diagnosed stage I non-small cell lung cancer underwent mediastinoscopy. Results: Two cases of N2 stage and one case of N3 stage (right anterior myxoid lymph node metastasis) were found in mediastinoscopy. The positive rate was 10% (3/31). The sensitivity was 75%, specificity was 100%, and accuracy Rate of 97%. Conclusion: The mediastinoscopy in stage Ⅰ clinical non-small cell lung cancer is of great significance. Preoperative mediastinoscopy can be routinely performed.
