洋地黄对充血性心力衰竭和大多数室上性心动过速是一种有效药物,但治疗安全范围较小。近年来中毒的发生率有所增加,洋地黄中毒反可加重心衰和引起各种心律失常。心律失常是洋地黄中毒死亡的重要原因。洋地黄的正性肌力作用(加强心肌收缩力)与毒性作用皆由跨膜正离子流改变所引起。此改变是洋地黄对 Na~+K~+—ATP 酶即洋地黄受体直接作用而产生的。因此洋地黄的正性肌力作用的亚细胞基础与中毒作用密切相关。
Digitalis is an effective drug for congestive heart failure and most supraventricular tachycardia, but it is less safe to treat. In recent years, the incidence of poisoning has increased, digitalis poisoning can aggravate heart failure and cause a variety of arrhythmias. Arrhythmia is an important cause of digitalis poisoning death. Digitalis positive inotropic effect (to enhance myocardial contractility) and toxic effects by the transmembrane positive ion current caused by changes. This change is the digitalis on Na ~ + K ~ + -ATP enzyme that digitalis produced by the direct effect. Therefore, digitalis positive inotropic effect of subcellular basis and toxic effects are closely related.
