目的探讨青岛市肾综合征出血热(HFRS)患者发热期淋巴细胞亚群和病毒载量的变化特点。方法 25例HFRS发热期患者设为HFRS发热期组,30例无偿献血者设为正常对照组。通过胶体金法检测HFRS发热期组的汉坦病毒免疫球蛋白M(Ig M)抗体;通过荧光定量聚合酶链反应(PCR)和流式细胞仪测定两组的淋巴细胞亚群和病毒载量。结果 HFRS发热期组血清中Ig M抗体阳性,其病毒载量为(69.8±27.4)copies/μl。HFRS发热期组总T细胞(CD3+)和T抑制/细胞毒细胞亚群(CD3+CD8+)百分比显著高于正常对照组,T辅助/诱导细胞亚群(CD3+CD4+)百分比和CD4/CD8 T细胞比值(CD4+/CD8+)及自然杀伤细胞(CD16+56+)百分比明显低于正常对照组,差异有统计学意义(P<0.01),但B细胞(CD3-CD19+)百分比与正常对照组比较差异无统计学意义(P>0.05)。结论总T细胞和T抑制/细胞毒细胞亚群百分比增高有利于HFRS患者早期的免疫应答。
Objective To investigate the changes of lymphocyte subsets and viral load in patients with HFRS during the period of fever in Qingdao. Methods Twenty-five patients with HFRS during fever were enrolled as HFRS fever group and 30 non-compensated blood donors as normal control group. Hantavirus immunoglobulin M (Ig M) antibody was detected by colloidal gold method in HFRS group. Lymphocyte subsets and viral load were measured by fluorescence quantitative polymerase chain reaction (PCR) and flow cytometry . Results Serum Ig M antibody was positive in the HFRS fever group with a viral load of (69.8 ± 27.4) copies / μl. The percentages of total T cells (CD3 +) and T-suppressor / cytotoxic subsets (CD3 + CD8 +) in HFRS fever group were significantly higher than those in the normal control group, the percentage of T helper / induced cell subsets (CD3 + CD4 +) and CD4 / CD8 T The percentages of CD4 + / CD8 + and CD16 + 56 + were significantly lower than those of normal control group (P <0.01), but the percentages of B cells (CD3-CD19 +) were significantly higher than those of normal control group The difference was not statistically significant (P> 0.05). Conclusion The increase of the percentage of total T cells and T-suppressor / cytotoxic subsets is beneficial to the early immune response in HFRS patients.
