我院于1993年11月至1996年9月共收治少儿髌骨软骨袖套式骨折4例,现报告如下。 1 临床资料 1.1 一般资料 本组4例,男3例,女1例;年龄10～16岁;右侧3例,左侧1例;受伤原因:跳远致伤2例,溜冰摔伤1例,跑步摔伤1例。新鲜骨折2例,陈旧性骨折2例,合并内侧支持带、股四头肌腱膜撕裂伤3例。陈旧性骨折均早期在外院多次摄片检查认为无骨折而被误诊,时间分别为1个半月、3个月。 1.2 治疗方法 4例病例中,2例新鲜骨折急诊行切开复位粗丝线髌骨周围环形缝扎固定,并修复撕裂的股四头肌腱膜及支持带。2例陈旧性骨折亦择期予手术,咬平骨折两断端,布巾钳钳夹复位,粗丝线髌骨周围双环缝扎固定,髌骨两侧缘软骨用4号丝线大圆针各缝合固定1针,股四头肌腱膜加固缝合,以确保关节面平整。术后膝部伸直位石膏托固定5～6周,逐步进行膝关节功能锻炼。

Old fractures are early in the hospital multiple radiographs that no fracture was misdiagnosed, time were 1 and a half months, 3 months. 1.2 Treatment 4 cases, 2 cases of fresh fractures underwent open reduction and reduction of the thick patella loop around the suture, and repair of the torn quadriceps tendon and support band. 2 cases of old fractures were also elective surgery, bite flat fracture two ends, cloth towel clamp reduction, thick silk thread around the patella double ring suture, both sides of the patella cartilage on the 4th with a large round needle stitches a needle, the stock Quadriceps tendon reinforcement suture to ensure that the articular surface formation. Postoperative knee extension plaster care fixed 5 to 6 weeks, and gradually knee functional exercise.
