破伤风是士的宁样长效毒素所致的中枢神经系统中毒症,其症状取决于破伤风毒素的药效学和药动学,致死的基本原因是呼衰及继发性心血管功能障碍。尽管在治疗上进展显著,但死亡率仍高达40~60%。有人试用鞘内或脑池内注射破伤风抗毒素或鞘内注射皮质激素,效果均不肯定。有人应用鞘内注入氯苯氨丁酸(巴氯芬;baclo-fen)治疗脊髓性或脑性痉挛,已获成功,作者认为,可能是由于抑制脊髓内神经递质的作用。作者报道2例破伤风病人,试用鞘内注射氯苯氨丁酸治疗,并根据肌电图的肌肉动作电位的时间—电压积分判定疗效。
